安全管理考核辦法HSEpenalties目錄content總則ChapteroneGeneral工作許可證ChaptertwoPermitToWork上鎖掛簽ChapterthreeLockoutandTagout事故報告與調(diào)查ChapterfourIncidentsreportingandinvestigation安全設(shè)備維護與檢查Chapterfivesafetyequipmentsmaintenanceandinspection應(yīng)急反應(yīng)ChaptersixEmergencyresponse安全臺賬管理ChaptersevenHSEfilesmanagement現(xiàn)場安全管理ChaptereightSiteHSEmanagement安全獎勵ChapternineHSEreward第一章：總則Chapterone:General為了規(guī)范我廠各級管理部門及全體員工的行為，安全高效的完成各項工作任務(wù)，使企業(yè)規(guī)范化、制度化、科學(xué)化，根據(jù)有關(guān)法律法規(guī)，行業(yè)標(biāo)準(zhǔn)和公司安全管理體系文件特制定本辦法。Inordertoimprovethebehaviorofallpersonnelonboard,ensureasafeandefficientcompletionofvarioustasks,andacontinuousimprovementofHSEmanagement.inaccordancewithrelevantlawsandregulationsofindustrystandardsandcompanyHSEMSthefollowingrulesareformulated.凡在我公司各工種崗位的所有員工，包括承包商，都必須切實按照本辦法的規(guī)定做好本職工種。TheserulesapplytoallpersonnelworkingatHanasLNGplant.第二章：工作許可證Chaptertwo:PTW所有人員必須遵守哈納斯液化天然氣工廠工作許可證制度，應(yīng)該申請工作許可而沒有申請的,對責(zé)任人罰款200元。在工作許可證發(fā)出之前開始工作的，以及在重新生效前進行工作的，都視為無票證作業(yè)。發(fā)現(xiàn)一次對作業(yè)主管罰款200元。AllpersonnelmustcomplywiththePermittoWorksystem.IfaPTWisnotappliedasrequired,afineof200RMBshallbeimposedonthepersonheldresponsible.Startingworkbeforetheworkpermitisissued/revalidatedwillbedeemedthesameasabove.Afineof200RMBshallbeimposedonthetasksupervisor.簽名視為唯一的授權(quán)確認，未經(jīng)書面允許代替他人簽名的，對責(zé)任人員罰款100元。SignatureisregardedastheonlyauthorizationofPTW.Afineof100RMBshallbeimposedonpersonheldresponsibleforsigningPTWforotherswithoutofficialdelegation.在拿到許可證之后工作之前須由作業(yè)主管召集所有作業(yè)人員召開工具箱會，沒有召開的，發(fā)現(xiàn)一次對作業(yè)主管罰款200元。Oncetheworkpermitisissued,priortocommencingthejob,Tasksupervisorshallhaveatoolboxmeetingwithallpeopleinvolved.Ifthetoolboxmeetingisnotheld,tasksupervisorshallbepenalizedwithafineof200RMB工作許可證應(yīng)定期由HSE專員組織生產(chǎn)和維修現(xiàn)場管理人員進行抽樣審核，未完成月度審核目標(biāo)，對HSE負責(zé)人罰款200元，無故拒不參加審核者罰款200元。HSEpersonnelshallfacilitatetheregularPTWauditsaccordingtoaagreedscheduleandtheproduction/maintenancesitemanagementteamshallbeinvolved.iftheschedulenotfollowed,HSEmanagershallimposeafineof200RMB.Thedesignatedpersonwhorefusetopresenthimselfshallbepenalizedwithafineof200.屬地負責(zé)人應(yīng)確認所有經(jīng)手的許可證不存在沖突作業(yè)，否則罰款200元。Arearesponsibleshallensurenoconflicts/interferencebetweenworksandotherconcurrentoperations.A200RMBfineshallbeimposediffailedtoindentifythelikelyrisks.作業(yè)主管應(yīng)檢查并確認該作業(yè)所用的所有工具與設(shè)備處于完好狀態(tài)，特別是對承包商的設(shè)備和工具。發(fā)現(xiàn)有缺陷的工具投入使用的，對作業(yè)主管罰款200元。Tasksupervisorshallcheckandensurethetoolsandequipmentsusedforthetasksareingoodworkingconditions,especiallyforthecontractor’stoolsandequipments.Ifanyunsafetoolsandequipmentsarefoundinuse,thetasksupervisorshallbepenalizedwithafineof200RMB.承包商工頭應(yīng)確保本組所有人員都已經(jīng)經(jīng)過哈納斯的入場培訓(xùn)，否則禁止申請任何工作許可。如有違反，對承包商工頭罰款200元。ContractorforemanshallensureeveryoneinhisgroupcompletedtheHanasLNGplantHSEinduction.Contractorforemanshallbeimposedafineof200RMBfortheviolation.高風(fēng)險作業(yè)的作業(yè)許可可能會要求安全監(jiān)護人在場。沒有按照作業(yè)程序指定安全監(jiān)護人的，對作業(yè)主管罰款200元，指定了安全監(jiān)護人，而沒有履行安全監(jiān)護人職責(zé)的，對指定的安全監(jiān)護人罰款200元。Highriskjobsmayrequiresafetywatcher/guard.Afineof200RMBshallbeimposediftasksupervisorfailtonominatethesafetywatcherinaccordancewithHSEMS.Afineof200RMBshallbeimposedifthenominatedsafetywatcher/guardfailtoperformhisduty.一些工作許可會要求測氣，沒有根據(jù)許可要求的檢測氣體，或者檢測頻率達不到要求的，對責(zé)任人罰款200元。Somejobsmayrequiregastesting.Failtoperformgastestingorthefrequencyofgastestingcan’tmeetthePTWrequirement,responsiblepartyshallbeimposedafineof200RMB屬地負責(zé)人和作業(yè)主管及承包商工頭應(yīng)檢查并確認現(xiàn)場工作準(zhǔn)備完全符合工作許可證關(guān)于安全作業(yè)的要求。如果工作開始后現(xiàn)場安全措施發(fā)現(xiàn)低于許可證的要求。對作業(yè)主管和承包商工頭每人罰款200元，對屬地負責(zé)人罰款100元。Arearesponsible,tasksupervisorandcontractorforemanshallcheckandensurethatallspecifiedrequirementslaiddowninthePTWareimplementedbeforestartingthework.IncasecontrolmeasuresspecifiedinPTWarefoundnotinplaceafterstartingthejob,tasksupervisorandcontractorforemanshallbeimposedafineof200RMB,andarearesponsibleshallbeimposedafineof100RMB.所有的工作許可在工作結(jié)束后應(yīng)立即關(guān)閉，未及時關(guān)閉者，對作業(yè)主管罰款200元。未遵守延時程序，暫停或者重新生效程序的，對作業(yè)主管罰款100元。Allpermitsshallbeclosedoncethejobisdone.Failtoclosethepermitsintime,tasksupervisorshallbepenalizedwithafineof200RMB.ForfailuretofollowthePTWextension,suspendandrevalidationprocedure,tasksupervisorshallbeimposedafineof100RMB已經(jīng)申請工作許可，但未在工作現(xiàn)場張貼的。對作業(yè)主管罰款50元。PTWisissuedbutnotpostedonsite,tasksupervisorshallbeimposedafineof50RMB作業(yè)主管，HSE經(jīng)理和屬地負責(zé)人必須參加日常協(xié)調(diào)會議，無故不參加者或者因有事無法參加而且沒有指定代理人的，罰款50元。Tasksupervisor,HSEmanagerandarearesponsibleshallattendthedailyPTWcoordinatingmeeting.Afineof50RMBshallbeimposedforabsencewithoutexcuseorabsencewithoutdelegation.所有發(fā)出的工作許可應(yīng)記錄在工作許可記錄簿中，發(fā)現(xiàn)記錄不全或者無記錄的，對當(dāng)值HSE專員罰款50元。Allpermitsissuedshallberecordedinthelogbook.IfLogbooknotmaintainedproperly,afineof50RMBshallbeimposedupondutyHSEengineer.作業(yè)結(jié)束后，在關(guān)閉工作許可前，作業(yè)主管或者承包商工頭必須確認工作現(xiàn)場清理干凈，工具設(shè)備歸位或者復(fù)位。如果違反本規(guī)定，對作業(yè)主管或者承包商領(lǐng)班罰款200元。Tasksupervisorand/orcontractorforemanshallensuretheworksiteisleftclean,andequipments,materialandanywastesmustberemovedpriortoclosetheworkpermit.Afineof200RMBshallbeimposeduponTasksupervisorforviolation.作業(yè)條件或者作業(yè)性質(zhì)發(fā)生改變，應(yīng)停止工作，重新申請作業(yè)許可。未及時停止作業(yè)的，如果有安全監(jiān)護人，對安全監(jiān)護人罰款200元。沒有安全監(jiān)護人的對作業(yè)主管或者承包商領(lǐng)班各罰款200元，并對屬地負責(zé)人罰款100元。WorkshallbestoppedandPTWshallbere-appliedoncetheworkconditionsorworknaturechanged.Safetywatcher,ifapplicable,shallbeimposedafineof200RMBforviolation.Tasksupervisor/contractorforeman,ifsafetywatcherisnotapplicable,shallbeimposedafineof200RMB,andarearesponsibleshallbeimposedafineof100RMBforviolation.每年對工廠所有長期員工進行一次工作許可程序的重新培訓(xùn)。未及時組織本培訓(xùn)的，對HSE負責(zé)人以及生產(chǎn)和維修負責(zé)人各罰款200元。AllHanasLNGemployeeandlongtermcontractorshallreceiveayearlyPTWrefreshertraining.Ifthistrainingisnotorganizedintime,afineof200RMBshallbeimposedonHSEmanger,productionmanagerandmaintenancemanager.第三章：上鎖掛簽Chapterthree:LOTOTO在進行作業(yè)之前，必須對將要進行的作業(yè)進行辨識，如果存在機器，設(shè)備意外的受能、啟動與工藝系統(tǒng)釋放意外危險能量的可能，必須進行能量隔離，通常是通過上鎖掛簽來實現(xiàn)的。未根據(jù)本規(guī)定進行上鎖掛簽或者其他適宜方式的能量隔離的，對所有當(dāng)事人罰款200元。Anyenergymaybeinvolvedduringtheworkmustbeeffectivelyidentified.Toavoidanyrisksofincidentsthatmayarisefromtheunexpectedenergization,start-up,orreleaseofhazardousenergywhenperformingtasksonmachines,equipmentsorprocessingsystem,energyisolationmustbedone.Mostlikely,energyisolationisachievedbyLOTO.IftheLOTOisnotdoneornotproperlydoneasperrequirement,afineof200RMBshallbeimposeduponpersonnelresponsiblefortheactivity.在能夠上鎖的的隔離點采用掛牌（代替上鎖）作為首要的能量隔離方法是不允許的。違者罰款200元。Itisanunacceptablepracticetoutilizeinformationtags(inlieuoflockout)astheprimarymethodofhazardousenergycontrolonisolationpointsthatcanbelocked-out.Afineof200RMBshallbeimposedfortheviolation.在特定情況下，一些特殊隔離點無法做到上鎖，經(jīng)設(shè)備負責(zé)人書面同意（在掛簽上背書可接受），可以只做掛簽處理。未經(jīng)設(shè)備負責(zé)人書面同意的，對執(zhí)行人罰款200元。Incaseswhereanisolationpointcannotbelocked-out,writtenapproval(endorsementontagisdeemedasawrittenapproval)mustbeobtainedfromequipmentresponsiblepersonforapplyinginformationtagonly.Afineof200RMBshallbeimposedfortheviolation.作業(yè)需要兩個以上（含兩個）隔離點上鎖時，必須使用集體鎖。即集體鎖用于能量隔離點，工作人員和隔離執(zhí)行人的個人鎖用于鎖集體鎖的鑰匙，而不是將個人鎖直接用于隔離點。違反本規(guī)定，對隔離執(zhí)行人罰款200元。Agrouplockingdevicemustbeusedwhenataskrequirestwoormoreisolationpoints.WhenapplyingthegroupLOTO,thegrouplockingdevicesareappliedattheisolationpoints,andtheindividuallockingdevicesareusedtolockoutthekeyofgrouplockingdevicesratherthanlocktheisolationpointsdirectly.Afineof200RMBshallbeimposedontheresponsiblepersonfortheviolation.任何上鎖，包括集體鎖站上的個人鎖，都必須附有掛簽，雖然已經(jīng)“上鎖”但是沒有掛簽的，或者掛簽填寫不完全的，對上鎖掛簽執(zhí)行人罰款50元。Aninformationtagmustbeattachedtolockingdevicesinuse.Ifinformationtagisnotinplaceortheinformationtagisnotfilledoutcorrectly,althoughthelock-outisdone,afineof50RMBshallbeimposedontheresponsibleperson.所有“上鎖”必須有本人解除。如果本人不在廠內(nèi)，須經(jīng)過上鎖人書面或者口頭同意并經(jīng)過上級主管批準(zhǔn)，才能解除。違反本規(guī)定者，罰款200元。Allthelock-outscanonlyberemovedbythepersonwhoappliedthisLOTO.Ifheisabsentfromsite,thepermissionfromhimortheapprovalfromhissupervisormustbeobtainedforremovingthelock-outbyothers.Afineof200RMBshallbeimposedforviolation.任何人不準(zhǔn)操作已經(jīng)上鎖掛簽的設(shè)備和系統(tǒng)，除非處于測試的目的。否則罰款200元。Donotoperatetheequipmentsbeinglockedoutand/ortaggedoutintentionallyunlessfortestingpurpose.Afineof200RMBshallbeimposedforviolation.隔離執(zhí)行人員應(yīng)在隔離完成之后進行測試，確認隔離效果。然后在隔離單上簽字，未測試或者未簽字的罰款200元。Tryoutmustbeperformedbyexecutionpersonaftertheisolationisdoneensuringtheeffectivenessofisolation.Andthensigntheisolationpermit.Afineof200RMBshallbeimposedonexecutionpersoniftryoutisnotdone,ortheisolationpermitisnotsigned.作業(yè)人員開始工作之前應(yīng)確認設(shè)備、系統(tǒng)已正確的上鎖、掛標(biāo)簽及測試，并在隔離單上簽字，未測試或者未簽字的，對作業(yè)人罰款200元。Maintenancepersonnelshalltestthelockedoutequipmentspriortoworkensuringtheeffectivenessofisolation.Andthensigntheisolationpermit.Afineof200RMBshallbeimposediftestingisnotdoneortheisolationpermitisnotsigned作業(yè)需要五個及以上隔離點上鎖時，必須附有PID，PID不可用時需要以草圖示意。違反本規(guī)定，對隔離方案編制人罰款100元。PIDoraschetch(ifPIDnotavailable)shallbeattachediftherearemorethan5isolationpointsinaisolationplan.Afineof100RMBshallbeimposedonisolationplannerforviolation.當(dāng)換班時工作尚未完成，設(shè)備所屬部門集體鎖、個人鎖及“危險禁止操作”標(biāo)簽不能被解除，接班人必須先用他的個人鎖鎖上設(shè)備所屬單位集體鎖箱并掛簽后，同時在工作票上簽字，交班人才可解除他的個人鎖和簽。未遵守本規(guī)定者，對交接班雙方罰款200元。Ifaworkhasnotbeendonewhilehandover,single/grouplockersshallnotberemoveduntilpeoplewhoaretakingoverappliedtheirownsingle/grouplockersandsignedtheworkpermit.Afineof200RMBshallbeimposedonbothpartiesifthisprocedurenotfollowed.第四章：事件事故報告與調(diào)查Chapterfour:nearmisses/Incidentsreportingandinvestigation在哈納斯天然氣工廠所有未遂事件及事故都必須報告。如果發(fā)現(xiàn)隱瞞行為，針對嚴重程度不同對當(dāng)事人及目擊者罰款100-2021元。AlltheincidentsandaccidentsatHanasLNGplantmustbereported.Dependsonthepotentialseverity,afineof100-2021EMBshallbeimposedforhidinganyincidents/accidents.本規(guī)定將人員受傷事故分為三個等級，輕微事故（簡單醫(yī)療事故，醫(yī)療事故），中等事故（單人未導(dǎo)致殘疾的損失工時事故和工作受限事故）和重大事故（多人損失工時事故，單人導(dǎo)致殘疾的損失工時事故，直至死亡事故）。輕微事故如果當(dāng)事人和責(zé)任人及時報告，將不做追究。中度事故經(jīng)過事故調(diào)查，確認責(zé)任人后，根據(jù)責(zé)任劃分分別給予500-1000元罰款。重大事故，經(jīng)過事故調(diào)查，確認責(zé)任人后由公司管理層做出最終的處罰決定。對于環(huán)境事故和財產(chǎn)損失事故將按照嚴重程度與上述相應(yīng)等級處理。Injuryincidentswillbeclassifiedinto3categoriesforthisrule:Minor(FA&MTC),moderate(singleLTIwithoutdisability&RWDC)andSerious(Multi-LTI,SingleLTIwithdisability,fatalityandabove).Nopenaltyactionsshallbetakentoagainstpeopleheldresponsibleifaminorinjuryhasbeenreportedintime.Afineof500-1000RMBshallbeimposedonwhomisjustifiedandprovedtobeheldresponsibleformoderateinjuryincidentthroughofficialincidentinvestigation.Thefinalpenaltydecisionagainstpersonheldresponsibleforseriousinjuryincidentshallbemadebyplantmanagementteamaftergoingthroughtheofficialinvestigation.ThesameprincipleasaboveappliestoenvironmentimpactandpropertydamageandbusinessinterruptionaccordingtoseveritymatrixstipulatedinHSEMS.所有人員必須積極配合事故調(diào)查，不得在事故調(diào)查中提供虛假信息，一經(jīng)確認將視為責(zé)任人同等處理。Allpersonnelmustactivelycooperatewiththeincidentinvestigationasrequired,andnofalseinformationshallbeprovided.Otherwise,thesameliabilitywithpeopleheldresponsibleforincidentshallbeimposed.哈納斯天然氣工廠將不安全狀況觀察卡做為一種簡單的不安全行為和不安全狀況的匯報工具，管理層鼓勵所有員工積極參與。HSE部門會制定每個人的月度目標(biāo)，對未完成的員工罰款10元。Asasimplifiedreportingtoolforunsafeconditionsandactions,HanasLNGmanagementteamencourageallpersonnelonboardtobeinvolvedinUSOCprogram.Monthlytargetissetforeveryemployee.Afineof10RMBshallbeimposedifthetargetisnotachieved.所有針對事故和不安全狀況的整改措施，都會由HSE部門和相關(guān)部門指定整改措施落實負責(zé)人和完成期限，未能在指定期限內(nèi)完成的，對整改措施負責(zé)人罰款50-300元。HSEdepartmenttogetherwithrelevantpartiesareresponsibletoassignincidentscorrectiveactionsownerandcompletiontargetdate.Allincidentscorrectiveactionsandhazardsrecognitioncorrectiveactionsshallbecompletedbeforetheagreedtargetdate.Afineof50-300RMBshallbeimposedoncorrectiveactionownersifthecorrectiveactionsarenotcompletedwithinthetimestipulated.環(huán)境事故和生產(chǎn)事故由生產(chǎn)部門負責(zé)協(xié)調(diào)事故調(diào)查。設(shè)備事故由維修部門負責(zé)事故調(diào)查。質(zhì)量事故由化驗室負責(zé)事故調(diào)查。人員受傷、安保事故、火災(zāi)事故和交通事故由HSE部門負責(zé)協(xié)調(diào)事故調(diào)查。事故調(diào)查協(xié)調(diào)人須在程序規(guī)定的時間內(nèi)提交事故調(diào)查報告。逾期未提交的，對對口部門負責(zé)人罰款200元。Theinvestigationofenvironmentdamageandproductionlossshallbefacilitatedbyproductiondepartment.Theinvestigationofpropertydamageshallbefacilitatedbymaintenancedepartment.Theinvestigationofqualityincidentshallbefacilitatedbylaboratory.Theinvestigationofinjuryincident,securityincident,fireincidentandvehicleincidentshallbefacilitatedbyHSEdepartment.TheincidentinvestigationfacilitatorshallsubmitthefinalreportwithinthetimestipulatedinHSEMS.Afineof200RMBshallbeimposedonthedisciplinemanagerfordelayofsubmittingthefinalreport.第五章：安全設(shè)備維護與檢查Chapterfive:Safetyequipmentsmaintenanceandinspection設(shè)備安全附件必須在第一次啟用前或者停用達到30天，重新啟用前進行測試。違反本條規(guī)定，對設(shè)備所屬部門負責(zé)人罰款200元。Safetysystemsaretobecheckedandtestedpriortotheirfirstuse;priortore-startupaftermorethan30daysshutdown;aftereachsafetysystemmodification.Afineof200RMBshallbeimposedtoequipmentresponsibledepartmentleaderforviolation.由于測試需要旁通安全設(shè)備的必須申請工作許可證。否則對作業(yè)人罰款50元。Permittoworkanddedicatedprocedureshallbeissuedeachtimerequiringanoverrideofsafetysystemorequipments.Afineof50RMBshallbeimposedforviolation.安全設(shè)備無法正常使用時，設(shè)備負責(zé)人需要立即報告并修理。24小時內(nèi)不能修復(fù)的，需要填寫安全降級登記表。未及時報告者，罰款200元；未填寫安全降級登記表者，罰款50元。Wheneveradefectonsafetyequipmentsisobserved,adefectreportshallbeissuedandtheequipmentsshallberepaired.Ifrepairisnotpossiblewithin24hrs,thesituationshallberecordedindefectsituationlistwithassociatedcompensatorymeasures.Ifnoreportmade,afineof200RMBshallbeimposed.Ifdefectsituationlistnotfilledout,afineof50RMBshallbeimposed.ESD系統(tǒng)每六個月進行一次不關(guān)停生產(chǎn)的測試并做好記錄。超過預(yù)定日期半個月未測試的，對責(zé)任人罰款200元。ESDshallbetestedeverysixmonthsbysimulatingabnormalconditionatthedetectorwithoutactuallyshuttingdowntheequipment.Theresultsshallberecordedclearly.Afineof200RMBshallbeimposedonresponsiblepersonformorethan15daysdelayofthistestornorecordsavailable.F&G探測系統(tǒng)每三個月進行一次不關(guān)停生產(chǎn)的測試并做好記錄。超過預(yù)定日期半個月未測試的，對責(zé)任人罰款罰款200元。Fireandgasdetectionsystemshallbetestedeverythreemonthswithoutproductionshutdown.Thetestresultsshallberecordedclearly.Afineof200RMBshallbeimposedonresponsiblepersonformorethan15daysdelayofsuchtestornorecordsavailable.便攜式氣體探測儀每月進行測試并記錄。超過預(yù)定日期一個星期未測試的，對化驗室負責(zé)人罰款100元。Portablegasdetectorsshallbecalibratedmonthly.Thecalibrationresultsshallberecordedclearly.Afineof100RMBshallbeimposedonresponsiblepersonformorethan7daysdelayofsuchcalibrationornorecordsavailable每周一當(dāng)班班組對柴油消防泵進行30分鐘的運行測試并記錄。未進行測試的，對當(dāng)班主管罰款200元。Ondutyoperationteamshallperforma30minutesrunningtestondieseldrivenfirefightingpumpandrecordthetestingresult.Afineof200RMBshallbeimposedondutysupervisorifthetestisnotdoneornorecordsavailable.每年對消防系統(tǒng)進行一次第三方檢測。超期三個月未做測試的，對HSE負責(zé)人罰款500元。Yearlyfirefightingsysteminspectionbythirdpartyisrequired.Afineof500RMBshallbeimposedonHSEmanagerformorethanthreemonthsdelayofsuchinspection,ornorecordsavailable.固定干粉滅火系統(tǒng)每六個月進行一次不關(guān)停生產(chǎn)的測試并記錄。超期一個月未測試的，對責(zé)任人罰款200元。Fixeddrypowderfirefightingsystemshallb
