8、re of the passengers at the destination 備注：此單一式四份，售票單位一份，始發(fā)站值機(jī)單位一份，旅客一份，到達(dá)站小動(dòng)物交付工作 人員一份。 北京首都航空有限公司小動(dòng)物運(yùn)輸申請(qǐng)書（背面） APPLICATION FOR PETS TRANSPORTION OF CAPITAL AIRLINES 一. 小動(dòng)物是指家庭馴養(yǎng)的小狗、貓。 二. 小動(dòng)物運(yùn)輸限制和條件 1、 提前 48 小時(shí)向首都航空售票單位進(jìn)行申請(qǐng)。 2、 除導(dǎo)盲犬、助聽犬及救助犬等工作犬外均不能作為客艙行李運(yùn)輸，必須辦理托運(yùn)。 3、 每名旅客限制僅能運(yùn)輸一只小動(dòng)物。 4、 小動(dòng)物作為托運(yùn)行李運(yùn)輸時(shí)

9、，寵物和籠子的總重量重量不得超過 20Kg。 5、 導(dǎo)盲犬、助聽犬、救助犬等工作犬運(yùn)輸不受本條 3 及 4 款規(guī)定限制。 6、 對(duì)于出現(xiàn)的以下情況，承運(yùn)人無法辦理航空運(yùn)輸：托運(yùn)人所托運(yùn)的活體動(dòng)物為國家禁運(yùn)的動(dòng)物之列。具有傳染 病征候，或疑似傳染病載體的動(dòng)物（如發(fā)生地區(qū)禽流感的家禽）。犬類動(dòng)物及貓科動(dòng)物年齡小于六周。懷孕動(dòng)物或是 在飛機(jī)起飛前 48小時(shí)之內(nèi)剛剛分娩過的動(dòng)物。需要托運(yùn)人完成的動(dòng)物飛行運(yùn)輸準(zhǔn)備未完成。運(yùn)輸過程中可能產(chǎn)生 的承運(yùn)人認(rèn)為的天氣、高熱（氣溫通常高于 85 F/29 C 的往返機(jī)場(chǎng)）等不適于哈巴狗或扁平鼻的狗或貓運(yùn)輸?shù)钠渌?原因。因航班機(jī)型原因，不適于運(yùn)輸活體動(dòng)物。（如貨艙無

10、適運(yùn)艙位）不符合始發(fā)、經(jīng)停、目的地城市或國家動(dòng)物 運(yùn)輸?shù)南嚓P(guān)規(guī)定航空公司工作人員判斷該動(dòng)物有可能對(duì)旅客和 /或其行李造成損害，或者航空運(yùn)輸有可能對(duì)該動(dòng)物造 成損害。 三、 運(yùn)輸文件要求 1、 乘坐國際航班，旅客應(yīng)在至少出發(fā)前 7 天為寵物備妥下列文件 相關(guān)政府（目的地/中轉(zhuǎn)地）核發(fā)的有效輸出/輸入文件。 有關(guān)當(dāng)局核發(fā)的有效健康聲明書及狂犬病疫苗注射證明書。 妥善的備齊所有行程中含蓋的入境/過境國要求的入境許可、健康聲明書及疫苗注射證明。 任何行程中含蓋的入境/過境國政府要求的額外特殊文件 2、 國內(nèi)運(yùn)輸所須文件 動(dòng)物衛(wèi)生監(jiān)督所出具的動(dòng)物檢疫合格證明，其中“運(yùn)載工具消毒情況”一欄必須填寫且單據(jù)上

11、需蓋有動(dòng)物衛(wèi)生監(jiān)督 所檢疫專用章。 3、 導(dǎo)盲犬、助聽犬、救助犬等工作犬運(yùn)輸所須文件：除須備齊本條 1 或 2 款證件外，還應(yīng)攜帶有效的 動(dòng)物訓(xùn)練合格證 明書、動(dòng)物工作證或/和動(dòng)物身份證。 四、 寵物箱要求 1、 首都航空建議旅客使用專用航空箱運(yùn)輸動(dòng)物。 2、 寵物箱必須由堅(jiān)固材料制成且頂部固定。乘坐國內(nèi)航班至少三面通風(fēng)，國際航班四面通風(fēng)，且門必須由金屬制成。 硬件（包括螺母、螺絲以及門）確保安裝完整。箱門牢固須有鎖閉裝置，有固定的把手或凸起邊緣。寵物箱的空間 應(yīng)足夠大，可以保證寵物自由站立或坐下、轉(zhuǎn)身和以正常姿勢(shì)躺臥。寵物箱的底部平穩(wěn)，能夠固定在平整的面上而 不滑動(dòng)。帶輪子的寵物箱，需要進(jìn)行

12、必要的固定。保證在運(yùn)輸過程中輪子不滑動(dòng) .寵物箱必須經(jīng)過打包處理，中號(hào)箱 每面至少捆扎 2 匝，呈“井”字形，大號(hào)箱每面至少捆扎 3 匝，打包帶分布均勻，保證在運(yùn)輸過程中正常情況下打 包帶不脫落。 3、 寵物箱箱內(nèi)須鋪上吸水性襯墊，例如毛巾、毯子或者白色的紙，以防止寵物排泄物外溢污染其他行李。 北京首都航空有限公司 1、 Scope of The Pets Transportation pets only limit to the domestic dogs ， cats . 2、 Limitations and Conditions of The Transportation (1) The

13、 passengers have to apply to the ticket agency of Capital Airlines at least 48 hours prior to departure. (2) Each passe ngers can only have one pet checked. (3) Except the service an imals, such as the see in g-eye dogs, heari ng-ear dogs and the rescue dogs, all the other pets are forbidde n to be

14、carried in the cab in, they must be tran sported as checked baggage. (4) Pets tran sported as checked baggage, the total weight of the pets and container cannot exceed 20Kgs. (5) The service an imals are exempt from the limitati ons of the item3 and 4. (6) Pets may be refused transportation in case

The checked pets belong to the forbidden transporting animals of the nation. pets with comm uni cable disease symptoms or suspected comm uni cable carrier, such as the poultry with regi onal bird flu The age of dogs and the cats is less tha n 6 weeks. The pets may be hurt during the transportation. 3、 The Necessary Documents for Transporting Pets 1) passe ngers should prepare the follow ing docume nts below at lea

19、st 7 days before the departure for in ter nati onal flight. Valid Certification for animals quarantine ， especially the vehicle disinfection situation must be filled and stamped with the official department. 2) Documents for the domestic transportation Valid Animals Immunity Certificate which should

20、 contain at least the rabies vaccine certificate Valid Quarantine Certificate for pets issued by China government. 21、sse ngers should also carry a valid Qualified Certificate for Pets Trai ning, Employees Card of The Ani mals an d/or the I.D. card. 4、The Requirements Of The Container. Capital Airlines suggests passengers use the special shipping container designed for transporting pets by air.
