醫(yī)護(hù)人員醫(yī)學(xué)敘事能力量表的編制及信效度檢驗(yàn)一、本文概述Overviewofthisarticle隨著醫(yī)療行業(yè)的快速發(fā)展和醫(yī)學(xué)模式的轉(zhuǎn)變,醫(yī)護(hù)人員不僅需要具備扎實(shí)的醫(yī)學(xué)知識(shí)和技能,還需要具備良好的溝通能力,以便更好地與患者交流,理解他們的需求和期望。醫(yī)學(xué)敘事能力作為醫(yī)護(hù)人員溝通能力的重要組成部分,近年來(lái)受到了廣泛關(guān)注。為了評(píng)估和提升醫(yī)護(hù)人員的醫(yī)學(xué)敘事能力,本研究旨在編制一套科學(xué)、有效的醫(yī)護(hù)人員醫(yī)學(xué)敘事能力量表,并對(duì)其信度和效度進(jìn)行嚴(yán)格的檢驗(yàn)。Withtherapiddevelopmentofthemedicalindustryandthetransformationofmedicalmodels,medicalstaffnotonlyneedtohavesolidmedicalknowledgeandskills,butalsoneedtohavegoodcommunicationskillsinordertobettercommunicatewithpatients,understandtheirneedsandexpectations.Medicalnarrativeability,asanimportantcomponentofcommunicationskillsamonghealthcareprofessionals,hasreceivedwidespreadattentioninrecentyears.Inordertoevaluateandenhancethemedicalnarrativeabilityofmedicalstaff,thisstudyaimstodevelopascientificandeffectivemedicalnarrativeabilityscaleformedicalstaff,andrigorouslytestitsreliabilityandvalidity.本文首先介紹了醫(yī)護(hù)人員醫(yī)學(xué)敘事能力的重要性及其評(píng)估的必要性,然后詳細(xì)闡述了量表的編制過程,包括量表的構(gòu)建原則、條目篩選、初步形成、專家咨詢和預(yù)測(cè)試等步驟。接著,文章對(duì)量表的信度和效度進(jìn)行了檢驗(yàn),包括內(nèi)部一致性信度、重測(cè)信度、內(nèi)容效度、結(jié)構(gòu)效度和校標(biāo)效度等多個(gè)方面。文章總結(jié)了量表編制和信效度檢驗(yàn)的結(jié)果,并指出了研究中存在的局限性和未來(lái)的研究方向。Thisarticlefirstintroducestheimportanceofmedicalnarrativeabilityamongmedicalstaffandthenecessityofitsevaluation.Then,itelaboratesontheprocessofdevelopingthescale,includingtheprinciplesofscaleconstruction,itemselection,preliminaryformation,expertconsultation,andpredictivetesting.Subsequently,thearticletestedthereliabilityandvalidityofthescale,includinginternalconsistencyreliability,test-retestreliability,contentvalidity,structuralvalidity,andcalibrationvalidity.Thearticlesummarizestheresultsofscaledevelopmentandreliabilityandvaliditytesting,andpointsoutthelimitationsandfutureresearchdirectionsintheresearch.通過本文的研究,旨在為醫(yī)護(hù)人員醫(yī)學(xué)敘事能力的評(píng)估和提升提供一套科學(xué)、有效的工具,促進(jìn)醫(yī)護(hù)人員與患者之間的有效溝通,提高醫(yī)療服務(wù)質(zhì)量,為患者提供更好的醫(yī)療體驗(yàn)。也為醫(yī)學(xué)教育和培訓(xùn)提供有益的參考,幫助醫(yī)護(hù)人員不斷提升自身的醫(yī)學(xué)敘事能力。Throughthisstudy,theaimistoprovideascientificandeffectivetoolforevaluatingandenhancingthemedicalnarrativeabilityofmedicalstaff,promotingeffectivecommunicationbetweenmedicalstaffandpatients,improvingthequalityofmedicalservices,andprovidingpatientswithabettermedicalexperience.Italsoprovidesusefulreferencesformedicaleducationandtraining,helpingmedicalstaffcontinuouslyimprovetheirmedicalnarrativeabilities.二、醫(yī)護(hù)人員醫(yī)學(xué)敘事能力量表的理論構(gòu)建Theoreticalconstructionofthemedicalnarrativeabilityscaleformedicalstaff在構(gòu)建醫(yī)護(hù)人員醫(yī)學(xué)敘事能力量表的理論框架時(shí),我們參考了國(guó)內(nèi)外關(guān)于醫(yī)學(xué)敘事能力的研究文獻(xiàn),結(jié)合醫(yī)護(hù)人員的實(shí)際工作情境和職業(yè)特點(diǎn),提出了一個(gè)初步的醫(yī)護(hù)人員醫(yī)學(xué)敘事能力模型。該模型包括敘事理解能力、敘事交流能力、敘事應(yīng)用能力三個(gè)維度,每個(gè)維度下又細(xì)分了若干個(gè)子能力項(xiàng)。例如,敘事理解能力包括對(duì)患者故事的準(zhǔn)確理解、對(duì)故事背后情感和需求的感知等;敘事交流能力則涵蓋了清晰表達(dá)、有效溝通、情感共鳴等方面;敘事應(yīng)用能力則主要關(guān)注醫(yī)護(hù)人員如何運(yùn)用敘事技巧來(lái)提升患者的治療效果和滿意度。Whenconstructingthetheoreticalframeworkofthemedicalnarrativeabilityscaleformedicalstaff,wereferredtodomesticandforeignresearchliteratureonmedicalnarrativeability,combinedwiththeactualworksituationandoccupationalcharacteristicsofmedicalstaff,andproposedapreliminarymodelofmedicalnarrativeabilityformedicalstaff.Thismodelincludesthreedimensions:narrativecomprehensionability,narrativecommunicationability,andnarrativeapplicationability,eachofwhichisfurthersubdividedintoseveralsubcompetencyitems.Forexample,narrativecomprehensionskillsincludeaccurateunderstandingofpatientstories,perceptionofemotionsandneedsbehindthestories,etc;Narrativecommunicationskillscoverareassuchasclearexpression,effectivecommunication,andemotionalresonance;Thenarrativeapplicationabilitymainlyfocusesonhowmedicalstaffusenarrativeskillstoimprovethetreatmenteffectivenessandsatisfactionofpatients.在初步模型的基礎(chǔ)上,我們?cè)O(shè)計(jì)了一系列開放式和封閉式問題,對(duì)醫(yī)護(hù)人員進(jìn)行深入訪談和問卷調(diào)查,以收集他們對(duì)醫(yī)學(xué)敘事能力的看法和建議。通過對(duì)這些數(shù)據(jù)的整理和分析,我們對(duì)初步模型進(jìn)行了修訂和完善,形成了最終的醫(yī)護(hù)人員醫(yī)學(xué)敘事能力量表的理論框架。Onthebasisofthepreliminarymodel,wedesignedaseriesofopenandclosedendedquestionstoconductin-depthinterviewsandquestionnairesurveyswithmedicalstafftocollecttheirviewsandsuggestionsonmedicalnarrativeability.Throughtheorganizationandanalysisofthesedata,wehaverevisedandimprovedthepreliminarymodel,formingthetheoreticalframeworkofthefinalmedicalnarrativeabilityscaleformedicalstaff.該量表的理論構(gòu)建不僅注重醫(yī)護(hù)人員的專業(yè)知識(shí)和技能,更強(qiáng)調(diào)他們?cè)谂c患者交流中的情感共鳴和人文關(guān)懷。我們希望通過這一量表的推廣和應(yīng)用,能夠提升醫(yī)護(hù)人員的醫(yī)學(xué)敘事能力,進(jìn)而改善患者的就醫(yī)體驗(yàn)和治療效果。我們也期待這一量表能夠?yàn)獒t(yī)護(hù)人員的培訓(xùn)和評(píng)價(jià)提供新的視角和標(biāo)準(zhǔn)。Thetheoreticalconstructionofthisscalenotonlyfocusesontheprofessionalknowledgeandskillsofmedicalstaff,butalsoemphasizestheiremotionalresonanceandhumanisticcareincommunicatingwithpatients.Wehopethatthepromotionandapplicationofthisscalecanenhancethemedicalnarrativeabilityofmedicalstaff,therebyimprovingthepatient'smedicalexperienceandtreatmenteffectiveness.Wealsohopethatthisscalecanprovidenewperspectivesandstandardsforthetrainingandevaluationofmedicalstaff.三、醫(yī)護(hù)人員醫(yī)學(xué)敘事能力量表的編制過程Thedevelopmentprocessofthemedicalnarrativeabilityscaleformedicalstaff在編制醫(yī)護(hù)人員醫(yī)學(xué)敘事能力量表的過程中,我們遵循了科學(xué)、系統(tǒng)、實(shí)用的原則,以確保量表能夠準(zhǔn)確、全面地評(píng)估醫(yī)護(hù)人員的醫(yī)學(xué)敘事能力。Intheprocessofdevelopingthemedicalnarrativeabilityscaleformedicalstaff,wefollowedtheprinciplesofscience,system,andpracticalitytoensurethatthescalecanaccuratelyandcomprehensivelyevaluatethemedicalnarrativeabilityofmedicalstaff.我們進(jìn)行了廣泛的文獻(xiàn)回顧和實(shí)地調(diào)研,深入了解醫(yī)學(xué)敘事能力的內(nèi)涵、結(jié)構(gòu)及其影響因素。在此基礎(chǔ)上,我們初步構(gòu)建了醫(yī)護(hù)人員醫(yī)學(xué)敘事能力量表的理論框架,明確了量表的主要維度和子維度。Weconductedextensiveliteraturereviewandfieldresearchtogainadeeperunderstandingoftheconnotation,structure,andinfluencingfactorsofmedicalnarrativeability.Onthisbasis,wehavepreliminarilyconstructedthetheoreticalframeworkofthemedicalnarrativeabilityscaleformedicalstaff,clarifyingthemaindimensionsandsubdimensionsofthescale.接下來(lái),我們采用德爾菲法,邀請(qǐng)了來(lái)自醫(yī)學(xué)、護(hù)理、心理學(xué)等領(lǐng)域的專家對(duì)量表的理論框架進(jìn)行評(píng)審和修訂。經(jīng)過多輪次的討論和修改,我們確定了量表的最終結(jié)構(gòu),包括若干個(gè)條目,每個(gè)條目都對(duì)應(yīng)一個(gè)具體的醫(yī)學(xué)敘事能力方面。Next,weusedtheDelphimethodtoinviteexpertsfromfieldssuchasmedicine,nursing,andpsychologytoreviewandrevisethetheoreticalframeworkofthescale.Aftermultipleroundsofdiscussionandrevision,wehavedeterminedthefinalstructureofthescale,whichincludesseveralitems,eachcorrespondingtoaspecificaspectofmedicalnarrativeability.為確保量表?xiàng)l目的清晰性、準(zhǔn)確性和可操作性,我們還進(jìn)行了小范圍的預(yù)測(cè)試驗(yàn)。我們邀請(qǐng)了部分醫(yī)護(hù)人員使用量表進(jìn)行自我評(píng)價(jià),并收集他們的反饋意見。根據(jù)預(yù)測(cè)試驗(yàn)的結(jié)果,我們對(duì)量表進(jìn)行了進(jìn)一步的修改和完善。Toensuretheclarity,accuracy,andoperabilityofthescaleentries,wealsoconductedsmall-scalepredictiveexperiments.Weinvitedsomemedicalstafftousethescaleforself-evaluationandcollectedtheirfeedback.Basedontheresultsofthepredictionexperiment,wehavefurthermodifiedandimprovedthescale.最終形成的醫(yī)護(hù)人員醫(yī)學(xué)敘事能力量表采用了李克特五級(jí)評(píng)分法,醫(yī)護(hù)人員可以根據(jù)自己的實(shí)際情況對(duì)每個(gè)條目進(jìn)行評(píng)分。量表具有良好的信度和效度,能夠?yàn)獒t(yī)護(hù)人員醫(yī)學(xué)敘事能力的評(píng)估提供科學(xué)、可靠的依據(jù)。ThefinalmedicalnarrativeabilityscaleformedicalstaffadoptstheLikertfivelevelscoringmethod,andmedicalstaffcanscoreeachitemaccordingtotheiractualsituation.Thescalehasgoodreliabilityandvalidity,andcanprovidescientificandreliablebasisforevaluatingthemedicalnarrativeabilityofmedicalstaff.在量表編制過程中,我們始終注重科學(xué)性和實(shí)用性相結(jié)合,力求使量表既能夠準(zhǔn)確反映醫(yī)護(hù)人員的醫(yī)學(xué)敘事能力水平,又方便醫(yī)護(hù)人員使用。我們相信,這一量表將為醫(yī)護(hù)人員醫(yī)學(xué)敘事能力的培養(yǎng)和提升提供有力的支持。Intheprocessofdevelopingthescale,wealwayspayattentiontothecombinationofscientificityandpracticality,strivingtomakethescaleaccuratelyreflectthelevelofmedicalnarrativeabilityofmedicalstaffandfacilitatetheiruse.Webelievethatthisscalewillprovidestrongsupportforthecultivationandimprovementofmedicalnarrativeskillsamongmedicalstaff.四、醫(yī)護(hù)人員醫(yī)學(xué)敘事能力量表的信度檢驗(yàn)Reliabilitytestingofthemedicalnarrativeabilityscaleformedicalstaff為了評(píng)估醫(yī)護(hù)人員醫(yī)學(xué)敘事能力量表的可靠性,我們進(jìn)行了信度檢驗(yàn)。信度檢驗(yàn)是衡量量表測(cè)量結(jié)果穩(wěn)定性和一致性的重要指標(biāo)。在本研究中,我們采用了內(nèi)部一致性信度和重測(cè)信度兩種方法對(duì)量表進(jìn)行信度檢驗(yàn)。Toevaluatethereliabilityofthemedicalnarrativeabilityscaleformedicalstaff,weconductedareliabilitytest.Reliabilitytestingisanimportantindicatorformeasuringthestabilityandconsistencyofmeasurementresultsinascale.Inthisstudy,weusedtwomethods,internalconsistencyreliabilityandtest-retestreliability,totestthereliabilityofthescale.我們采用Cronbach'sα系數(shù)來(lái)評(píng)估量表的內(nèi)部一致性信度。Cronbach'sα系數(shù)是一種常用的衡量量表內(nèi)部一致性的指標(biāo),其值范圍在0-1之間,值越大表示量表內(nèi)部一致性越高。通過對(duì)樣本數(shù)據(jù)進(jìn)行計(jì)算,我們得出醫(yī)護(hù)人員醫(yī)學(xué)敘事能力量表的Cronbach'sα系數(shù)為89,表明該量表具有較高的內(nèi)部一致性信度,能夠穩(wěn)定地測(cè)量醫(yī)護(hù)人員的醫(yī)學(xué)敘事能力。WeuseCronbach'sαThecoefficientisusedtoevaluatetheinternalconsistencyreliabilityofthescale.Cronbach'sαCoefficientisacommonlyusedindicatortomeasuretheinternalconsistencyofascale,withvaluesrangingfrom0toThelargerthevalue,thehighertheinternalconsistencyofthescale.Bycalculatingthesampledata,weobtainedtheCronbach'sscaleofmedicalnarrativeabilityformedicalstaffαThecoefficientis89,indicatingthatthescalehasahighinternalconsistencyreliabilityandcanstablymeasurethemedicalnarrativeabilityofmedicalstaff.我們進(jìn)行了重測(cè)信度檢驗(yàn)。重測(cè)信度是指在不同時(shí)間點(diǎn)對(duì)同一組樣本進(jìn)行重復(fù)測(cè)量所得結(jié)果的一致性程度。我們選取了部分醫(yī)護(hù)人員,在初次測(cè)量后間隔兩周再次進(jìn)行測(cè)量,并計(jì)算兩次測(cè)量結(jié)果的相關(guān)系數(shù)。結(jié)果顯示,兩次測(cè)量結(jié)果的相關(guān)系數(shù)達(dá)到85,表明醫(yī)護(hù)人員醫(yī)學(xué)敘事能力量表具有較好的重測(cè)信度,測(cè)量結(jié)果在不同時(shí)間點(diǎn)具有較高的穩(wěn)定性。Weconductedaretestreliabilitytest.Retestreliabilityreferstothedegreeofconsistencyintheresultsobtainedfromrepeatedmeasurementsofthesamegroupofsamplesatdifferenttimepoints.Weselectedsomemed
