To prevent them it is vital that there should be safe systems of work. They will often be applied and en

4、forced by means of a formal Permit to Work system.施工現(xiàn)場(chǎng)充滿(mǎn)了高強(qiáng)度的作業(yè)，因此發(fā)生重大事故的可能性也比較高。為了防患于未然，必須制定一套安全作業(yè)系統(tǒng)。其中用得比較多的是一種正式制度是作業(yè)許可證制度。1.3 All Critical Operations will be under the control of a Permit to Work system operated by the Site. Specific training will be given to all employees on the PTW system .All

8、 For example a work shop or grit blasting area where hot work is considered “normal operation” will not require Permit to Work. However these activities must be covered under other SSPs or special procedure for the area. Any other work that is not covered under the areas procedure will require a Per

RESPONSIBILITIES 職責(zé)3.1 The ConstructionHSE Manager is responsible for ensuring that this procedure is implemented. He is also res

14、.3 To operate the permit issue point to reflect the current state of work及時(shí)解決作業(yè)許可證制度在當(dāng)前執(zhí)行過(guò)程中出現(xiàn)的各種問(wèn)題。3.4.4 To provide a quality check on the completion of permits在許可證的任務(wù)完成以后，負(fù)責(zé)檢查作業(yè)質(zhì)量。3.4.5 To be the focal point for PTW at the location, and generate a sense of ownership in those individuals who work w

PROCEDURE程序4.1 General概述A Permit to Work system is a formal written system used to control certain types of work, which are identified as potentially hazardous. Essential feature

17、s of a Permit to Work system are:作業(yè)許可證制度是一種對(duì)某些類(lèi)型的作業(yè)進(jìn)行控制的正式書(shū)面制度。這類(lèi)作業(yè)一般是指那些具有潛在危險(xiǎn)的活動(dòng)。作業(yè)許可證制度的基本特征為：a. Clear identification of who may authorize particular jobs,明確地規(guī)定誰(shuí)有權(quán)批準(zhǔn)某些特殊的作業(yè)，b. Training and instruction in the issue and use of permits.就作業(yè)許可證的頒發(fā)和使用進(jìn)行必要的培訓(xùn)和指導(dǎo)。c. Monitoring and auditing to ensure that

18、 the system works as intended.對(duì)作業(yè)許可證的使用進(jìn)行監(jiān)督和審核，確保制度的實(shí)施滿(mǎn)足預(yù)期的目標(biāo)。4.2 Permit Display許可證上的顯示內(nèi)容4.2.1 Copies of a Permit to Work should be clearly displayed:應(yīng)該在下列場(chǎng)合清楚地顯示作業(yè)許可證的內(nèi)容：a) At the work site.在作業(yè)場(chǎng)所。b) In the permit co-ordination room, with additional copies at the worksite.在許可證辦公室，留有作業(yè)現(xiàn)場(chǎng)許可證的副本。4.3 Pe

19、rmit Suspension許可暫時(shí)中止4.3.1 If for any reason work controlled by a permit has to be suspended then the permit will immediately be returned to the Superintendent who will acknowledge the fact and co-ordinate the return of all permits to the Permit Office.如果由于各種原因，原來(lái)受許可證控制的某項(xiàng)作業(yè)不得不暫時(shí)停止, 則應(yīng)該將許可證立即返還所在部門(mén)的

20、主管，再由主管向許可證辦公室通報(bào)真相，并將各種許可證退還給許可證辦公室。4.3.2 Example of reasons for suspension are:許可證暫時(shí)收回執(zhí)行的理由舉例如下：a) Site general alarm.現(xiàn)場(chǎng)出現(xiàn)一般的報(bào)警。b) Operational reasons.操作原因。c) Conflict of work activities.作業(yè)發(fā)生沖突。d) Expiry of period for which permit is valid.作業(yè)許可證有效期到期。e) Change in work site conditions.作業(yè)現(xiàn)場(chǎng)的情況發(fā)生變化4.3

21、.3 After suspension it will be necessary, once permission has been given to recommence work, to have the permit re-validated before the work can restart.在許可證暫時(shí)收回以后，一旦允許重新開(kāi)始作業(yè)，則在重新開(kāi)始作業(yè)之前，必須對(duì)原頒發(fā)的許可證重新核準(zhǔn)。4.4 Cross Reference交叉確認(rèn)4.4.1 It is important to make sure that one activity under a Permit to Work

22、does not create danger for another ongoing activity.在根據(jù)作業(yè)許可證進(jìn)行某項(xiàng)作業(yè)時(shí)，必須確保不對(duì)其它正在進(jìn)行中的工作構(gòu)成威脅。這一點(diǎn)是非常重要的。4.4.2 The site HSE team should be aware of potential interaction and should check when a permit is prepared whether it impinges on one already issued.現(xiàn)場(chǎng)HSE小組應(yīng)該清楚地意識(shí)到各種作業(yè)之間可能存在的關(guān)聯(lián)關(guān)系，在準(zhǔn)備簽發(fā)許可證時(shí)，應(yīng)該仔細(xì)確認(rèn)它是否與

23、此前頒發(fā)的許可作業(yè)之間存在著沖突。4.5 Permit Revalidation許可證的重新核準(zhǔn)4.5.1 When a permit is authorized by site HSE team, it will have a time expiry placed on it. This may vary form a few hours to a full shift, dependingupon the nature of the work being undertaken and the frequency at which the activity is repeated.現(xiàn)場(chǎng)HSE

24、小組在簽發(fā)許可證時(shí)，應(yīng)該注明許可證的有效時(shí)間。有效期有可能是幾個(gè)小時(shí)，也有可能是整個(gè)班次，具體要由作業(yè)的性質(zhì)以及作業(yè)重復(fù)的頻率決定。4.5.2 If, at the end of this time period, the work being controlled by the permit is not complete the permit may be revalidated by the responsible person, who shall ensure, together with the site HSE team, that the original conditions

27、4.7.1 Duties are placed on various persons with regard to the operation of a Permit to Work system. 31、Permit to Work systems is necessary for all persons who may be involved.
