1、 上海市恒通路360號(hào)一天下大廈17C03室 電話：(021)63809585 FAX：(021)63809685 郵政編碼：200070 各種所需證明材料證明材料一般出自有關(guān)單位，文字應(yīng)規(guī)范、嚴(yán)謹(jǐn)，并加蓋有效印章。申請(qǐng)者應(yīng)提供的證明材料主要有以下幾種：1 學(xué)歷、學(xué)位證明，主要是大學(xué)、研究生畢業(yè)證書，碩士學(xué)位證書，博士學(xué)位證書；2.成績(jī)單，成績(jī)單是任何申請(qǐng)人都必須提供的，我國大專院校的成績(jī)?cè)u(píng)定大多采用百分制，而美國則以采用等級(jí)制為多，其平均值通常用平均成績(jī)點(diǎn)數(shù)（GPA）表示，因此，必須注明所采用的記分方法，最好能提自己在班級(jí)所排名次，現(xiàn)在，美國大多數(shù)學(xué)校都要求申請(qǐng)者通過ETS直接提供TOEFL
