1、醫(yī)療安全的反思與對(duì)策醫(yī)療事故處理?xiàng)l例頒布實(shí)施后1年來(lái),我院組織全院職工深刻領(lǐng)會(huì)新條例的精神,確保新條例的順利實(shí)施。始終把醫(yī)療安全放在重要位置,未發(fā)生一起重大醫(yī)療事故。我們主要從以下幾方面抓好該項(xiàng)工作。1 不斷強(qiáng)化職工的醫(yī)療安全防范意識(shí),加強(qiáng)醫(yī)療安全教育醫(yī)院領(lǐng)導(dǎo)不厭其煩地進(jìn)行全員醫(yī)療安全教育。多次聘請(qǐng)自治區(qū)醫(yī)院和兵團(tuán)醫(yī)院多位管理專家進(jìn)行專題講座,并通過(guò)院周會(huì)等場(chǎng)合強(qiáng)調(diào)醫(yī)療安全。使全體員工認(rèn)識(shí)到:醫(yī)療安全是醫(yī)療質(zhì)量的前提和最基本的要求,是醫(yī)療質(zhì)量的集中體現(xiàn),也是醫(yī)療管理水平、技術(shù)水平和服務(wù)水平的集中體現(xiàn)。沒(méi)有醫(yī)療安全,就談不上醫(yī)療質(zhì)量。凡是涉及患者安全的問(wèn)題,都是醫(yī)療安全問(wèn)題。因此,對(duì)醫(yī)療安全的認(rèn)
2、識(shí)要高于對(duì)一般醫(yī)療質(zhì)量的認(rèn)識(shí);醫(yī)院還發(fā)放有關(guān)醫(yī)療事故的各種學(xué)習(xí)資料,召開(kāi)了動(dòng)員大會(huì),要求各科室對(duì)照有關(guān)學(xué)習(xí)內(nèi)容,查找本科室的不安全因素,提出整改措施。對(duì)全院醫(yī)務(wù)人員進(jìn)行醫(yī)療安全知識(shí)考核,通過(guò)“潤(rùn)物細(xì)無(wú)聲,滴滴入心田”的培育,使醫(yī)務(wù)人員在醫(yī)療活動(dòng)中有“如履薄冰”的警覺(jué),切實(shí)做到警鐘長(zhǎng)鳴、常抓不懈。增強(qiáng)防范意識(shí),重塑白衣天使的良好社會(huì)形象。2 教育全院職工樹(shù)立新的醫(yī)療安全觀醫(yī)療安全不僅是業(yè)務(wù)、工作概念,還是依法行醫(yī)、依法行政的衛(wèi)生法制觀念。必須從新的高度、新的角度認(rèn)識(shí)醫(yī)療安全問(wèn)題。從法制建設(shè)、道德建設(shè)、機(jī)構(gòu)和隊(duì)伍建設(shè)、制度建設(shè)等多方面入手,強(qiáng)化醫(yī)療安全管理。健全有關(guān)規(guī)章制度,加強(qiáng)醫(yī)務(wù)人員醫(yī)療衛(wèi)生管
3、理法律、法規(guī)、規(guī)章、診療護(hù)理規(guī)范、常規(guī)和醫(yī)德醫(yī)風(fēng)教育,不斷強(qiáng)化醫(yī)務(wù)人員“三基”、“三嚴(yán)”基本功訓(xùn)練,堅(jiān)持各項(xiàng)行之有效的規(guī)章制度,如首診負(fù)責(zé)制、三級(jí)醫(yī)師查房制度、會(huì)診制度、疑難病例討論制度和院內(nèi)感染的監(jiān)控制度等,精心施行技術(shù)操作,以收到治病救人的最佳效果。醫(yī)療規(guī)程和制度就是醫(yī)院工作的法規(guī),要從嚴(yán)執(zhí)法,隨查隨糾,讓大家從服務(wù)態(tài)度、服務(wù)程序、服務(wù)質(zhì)量等方面查找內(nèi)部原因,增強(qiáng)預(yù)防意識(shí)。醫(yī)務(wù)科定期對(duì)運(yùn)行病歷和死亡病例,門診、住院處方進(jìn)行審閱檢查,嚴(yán)格規(guī)范醫(yī)療行為,保證醫(yī)護(hù)質(zhì)量、保障醫(yī)療安全,對(duì)出現(xiàn) 差錯(cuò)或事故者嚴(yán)格執(zhí)行處罰制度。加強(qiáng)醫(yī)療安全報(bào)告制度,做到重大醫(yī)療事件立即報(bào)告,嚴(yán)重差錯(cuò)及時(shí)報(bào)告,一般差錯(cuò)如
4、實(shí)報(bào)告,安全醫(yī)療報(bào)表每月5日以前報(bào)醫(yī)務(wù)科。查出因違規(guī)診療而造成嚴(yán)重醫(yī)療安全問(wèn)題的,一定要不講情面,嚴(yán)肅處理,決不姑息,盡一切可能防范措施杜絕醫(yī)療事故。3 以人為本,堅(jiān)持以病人為中心,增強(qiáng)服務(wù)意識(shí)把行風(fēng)評(píng)議與個(gè)人勞務(wù)費(fèi)、年終考核和先進(jìn)評(píng)比結(jié)合起來(lái),建立自我約束、自我控制、自我監(jiān)督機(jī)制,引導(dǎo)全體醫(yī)務(wù)人員,自覺(jué)把病人的難處作為服務(wù)的重點(diǎn),把病人的需要作為服務(wù)的內(nèi)容,把病人是否滿意作為衡量服務(wù)質(zhì)量的尺度;要求使用貴重藥物和超出公費(fèi)醫(yī)療標(biāo)準(zhǔn)的藥物和檢查,必須征得患者或家屬同意方可使用。4 充實(shí)和加強(qiáng)門、急診技術(shù)力量配備醫(yī)院以門、急診的“窗口”作為塑造醫(yī)院形象的工作重點(diǎn),充實(shí)和加強(qiáng)門、急診技術(shù)力量配備,并
5、抓住醫(yī)療工作的首要環(huán)節(jié),及時(shí)準(zhǔn)確診治病人,提高疑難病人、重癥病人首次確診率,減少患者復(fù)診率。為方便群眾就醫(yī),醫(yī)院還簡(jiǎn)化傳統(tǒng)的醫(yī)療流程。同時(shí),對(duì)急診人員進(jìn)行“充電”,徹底改變過(guò)去那種急診不急的問(wèn)題,使醫(yī)院“綠色通道”暢通無(wú)阻,提供快速、便捷、有效的全程優(yōu)質(zhì)服務(wù)。5 抓院感管理降低院感率,減少漏報(bào)率,健全院內(nèi)感染管理三級(jí)網(wǎng)絡(luò),加強(qiáng)對(duì)重點(diǎn)科室、重點(diǎn)人群的監(jiān)控,綜合分析院內(nèi)感染原因,有效控制院內(nèi)感染流行。6 做好醫(yī)療糾紛患者一方的接待工作,化解醫(yī)患矛盾醫(yī)務(wù)科建立群眾來(lái)訪登記本,做到及時(shí)記錄在案,及時(shí)調(diào)查處理,耐心地進(jìn)行解釋,盡力做到使患者滿意。醫(yī)療安全工作是一個(gè)長(zhǎng)期的、復(fù)雜的、變化的系統(tǒng)工程,實(shí)踐證明
