1、無明新科技大學(xué)旅館事業(yè)管理系建教合作合約書明新科技大學(xué)旅館事業(yè)管理系建教合作合約書明新科技大學(xué) (以下簡稱甲方)承(以下簡稱乙方)之協(xié)助,合作培育旅館人力資本,提供學(xué)生校外實(shí)習(xí)機(jī)會(huì)訓(xùn)練專業(yè)技能,從而使學(xué)生成為旅館服務(wù)及管理能力的人才,為明確雙方之權(quán)利義務(wù)關(guān)系,雙方承諾共同遵守下列各項(xiàng)條款:一、甲、乙雙方組成實(shí)習(xí)輔導(dǎo)小組,由明新科技大學(xué)旅館事業(yè)管理系實(shí)習(xí)委員會(huì)代表暨乙方代表共同組成,實(shí)習(xí)輔導(dǎo)小組任務(wù)如下:1.召開協(xié)調(diào)會(huì),解決學(xué)生實(shí)習(xí)問題或討論雙方合作事宜,以利甲方教學(xué)與乙方營運(yùn)的配合。2.實(shí)習(xí)期間甲方分發(fā)學(xué)生至乙方各單位實(shí)習(xí), 實(shí)習(xí)學(xué)生除接受甲方實(shí)習(xí)委員會(huì)管理外,亦接受乙方實(shí)習(xí)單位主管之指揮監(jiān)督

2、,遵守公司政策及工作規(guī)則。3.實(shí)習(xí)期間雙方派專人負(fù)責(zé)指導(dǎo)與考核,藉以督促學(xué)生校外實(shí)習(xí)的能力訓(xùn)練。4.乙方負(fù)責(zé)監(jiān)督學(xué)生實(shí)習(xí)訓(xùn)練與生活輔導(dǎo),如遇情節(jié)重大情事,得由雙方開會(huì)議定提前終止實(shí)習(xí),并通知家長協(xié)同處理。5.其它有關(guān)建教合作校外實(shí)習(xí)事項(xiàng)。二、實(shí)習(xí)期間:自民國年月日起至年月日止。三、甲方之職責(zé):1.協(xié)助乙方遴選分發(fā)實(shí)習(xí)學(xué)生。2.協(xié)助乙方實(shí)習(xí)訓(xùn)練內(nèi)容或課程,訪視輔導(dǎo)實(shí)習(xí)學(xué)生,提供學(xué)生考核資料。3.通知實(shí)習(xí)學(xué)生接受乙方半年單位輪調(diào)安排。4.負(fù)責(zé)約束選派之實(shí)習(xí)學(xué)生,切實(shí)遵守乙方所安排實(shí)習(xí)單位工作及作息規(guī)定。四、乙方之職責(zé):1.提供甲方實(shí)習(xí)學(xué)生員額與專業(yè)訓(xùn)練課程, 唯不使學(xué)生擔(dān)任非相關(guān)及危險(xiǎn)性工作。2.

3、安排甲方實(shí)習(xí)學(xué)生半年單位或職務(wù)輪調(diào), 并提前通知學(xué)生與甲方實(shí)習(xí)委員會(huì)訪視老師,但實(shí)習(xí)學(xué)生于柜臺(tái)、訂房、與餐廳內(nèi)場(chǎng)三項(xiàng)職務(wù)得免除輪調(diào)。3.非約定之學(xué)生實(shí)習(xí)單位異動(dòng),須征求本系實(shí)習(xí)委員會(huì)同意。4.實(shí)習(xí)期間負(fù)責(zé)實(shí)習(xí)學(xué)生工作時(shí)間內(nèi)之生活管理與實(shí)習(xí)成績之考核。5.實(shí)習(xí)期間給予實(shí)習(xí)學(xué)生每名每月新臺(tái)幣元正之實(shí)習(xí)津貼,并依勞動(dòng)基準(zhǔn)法及其它相關(guān)法令為實(shí)習(xí)學(xué)生辦理勞保、健保。6.實(shí)習(xí)期間應(yīng)給予實(shí)習(xí)學(xué)生比照一般職員之休假與福利制度。7.其它有關(guān)實(shí)習(xí)事項(xiàng),依勞動(dòng)基準(zhǔn)法與乙方人事規(guī)定辦理。五、本合約如有未盡事宜或變更事項(xiàng),由雙方協(xié)調(diào)修訂之。六、本合約自雙方簽約日起實(shí)施。無立約單位甲方:學(xué)校名稱:明新科技大學(xué)代 表 人:

5、ogy (MUST)(hereinafter calledCollege). Both parties hereby agreed to cooperate in the education and training ofhospitality students believing that this co-operation would benefit all parties.It is mutually agreed as follows:1.Sponsor will provide the attachment opportunity & training facilities to p

6、ractical forthe students from the College. List of EWM Industry Training Partners are asfollows:1)The Oriental Hotel, 2) Royal Plaza on Scotts, 3) Le Meridien Singapore, 4) TheRitz-Carlton Millenia, Singapore, 5) The Traders Hotel, 6) Plaza Hotel Parkroyal, 7)Negara on Claymore, 8) Swissotel The Sta

7、mford, 9) Raffles The Plaza.2.The attachment of the students is subject to Sponsor obtaining legal permits for thestudents.3.Period of training attachment is 12 months as agreed between Sponsor & College.4.College undertakes that every applicant is a bona fide registered student of theCollege and sh

8、all provide for each student the following documentation forapplying the legal permit:a) Letter of Information in accordance with Sponsors format.b) Latest Academic Record or Certificate.c) Photocopy of passport.d) Original pre-deployment medical fitness certificate.5.Sponsor will provide the studen

9、ts with the following conditions during attachment:a) Minimum MonthlyAllowance of S$600 per month.b) Free standard accommodation, duty meals, uniform & uniform laundry.c) Medical consultation by companys doctor for work related illness or injuries.Personal medical is students own cost.d) Supervision

10、 and guidance of each student during the training.e) On satisfactory completion, issue a testimonial and certificate to the students.6.The College shall be responsible for the following:a) Recommending selected students based on performance achievement indexand fluent English.b) Repatriate any stude

11、nts who commit during the attachment period anycriminal offence, seriousmisconduct or violates the rules & regulations ofSponsor or Employer or if unfit for training.c) College undertakes to impose severe punitive discipline on student terminatedand repatriated for College shall provide the Sponsor,

12、 without any demand,written information of any disciplinary action.無d) Recover from student & reimburse Sponsor the $5000 immigration SecurityGuarantee, if forfeited by Singapore Government, for any trainee whocommitted and convicted of Immigration offence.e) Inform the students their responsibiliti

13、es during the internship period and theirown expenses for hospitalization & personal insurance and airfares to/fromSingapore.7.The College and Sponsor shall not be responsible for any misconduct, accident,injury or death of anystudent during attachment.8.The Sponsor may terminate the training and re

14、patriate any student at the studentsexpense for any ofthe following reason:a) Commit any Work Permit offences or involved in any conduct or activitiesincompatible or improper with this Training Program.b) Not complying with the rules and regulations of Sponsor, employer or the lawsof Singapore.c) Ce

15、rtified medically unfit by Sponsors appointed doctor or is incapacitated bysickness or diseases.d) Poor performance or conduct.9.The duration of this Agreement is valid for a period of one and a half (1.5) yearsupon signing. During this period, the College undertakes to send students forInternship o

16、r training attachment in Singapore to the Sponsor only and shall notsend any students directly or indirectly to any other party, employer orestablishment in Singapore without the written agreement of the Sponsor.10. The employment of the Students and this Agreement shall be governed solely bythe law

18、簽約之建教合作實(shí)習(xí)代表EWM Consultancy PteLtd 安排第三學(xué)年 校外實(shí)習(xí) 課程之海外專業(yè)實(shí)習(xí), 為規(guī)范 EWM Consultancy PteLtd 與簽署人間之權(quán)益,簽署人與實(shí)習(xí)生同意下列條款并遵守履行之。1.實(shí)習(xí)單位部門:EWM Consultancy Pte Ltd 安排之新加坡國際級(jí)觀光飯店2.月薪:600 新加坡幣3.每周工作時(shí)數(shù):48 小時(shí)4.實(shí)習(xí)時(shí)間:12 個(gè)月5.實(shí)習(xí)績效保證金:實(shí)習(xí)生應(yīng)于抵達(dá)新加坡后繳交$500 元新加坡幣給 EWMConsultancy Pte Ltd 作為實(shí)習(xí)績效保證金,實(shí)習(xí)生完成一年實(shí)習(xí)后將全額退回;若未完成一年實(shí)習(xí),此實(shí)習(xí)績效保證金將

19、作為 EWM Consultancy Pte Ltd名譽(yù)受損與實(shí)習(xí)安排經(jīng)費(fèi)之賠償。6.實(shí)習(xí)生合約完成之績效獎(jiǎng)金$100 新加坡幣每月, 績效獎(jiǎng)金于實(shí)習(xí)完成后一次發(fā)給;未完成 12 個(gè)月實(shí)習(xí)合約,無法領(lǐng)取績效獎(jiǎng)金。7.實(shí)習(xí)期間提供免費(fèi)宿舍,每間房間住四位實(shí)習(xí)生。實(shí)習(xí)生須負(fù)責(zé)平均分?jǐn)偯吭滤M(fèi)、電費(fèi)、瓦斯費(fèi)及宿舍受損修護(hù)等費(fèi)用;工作時(shí)由實(shí)習(xí)單位提供免費(fèi)餐食。8.若未獲機(jī)關(guān)補(bǔ)助,則實(shí)習(xí)生自行負(fù)擔(dān)新加坡來回機(jī)票費(fèi)與每日所需之交通費(fèi)。9.任何一方若決定終止實(shí)習(xí)關(guān)系, 須于一個(gè)月前以書面提出或支付一個(gè)月的工作薪資;在實(shí)習(xí)工作期間與工作之后若有行為不當(dāng)、表現(xiàn)不佳、觸犯法律、違反規(guī)定或與顧客交往,實(shí)習(xí)單位有權(quán)終

20、止實(shí)習(xí)合作關(guān)系且無須提前告知并遣送回國,由實(shí)習(xí)生自行負(fù)責(zé)回國交通費(fèi)。10. 上班工作時(shí)間完全由實(shí)習(xí)單位安排,實(shí)習(xí)生可以在同一個(gè)工作單位或不同部門工作。除了實(shí)習(xí)單位外,實(shí)習(xí)生不得在新加坡其它單位工作或打工,亦不得吸毒、酗酒、從事不法、不道德、不誠實(shí)、犯罪或政治活動(dòng)等違反新加坡法律之行為。11.除了 EWM Consultancy Pte Ltd 安排的宿舍外,實(shí)習(xí)生不可以在外過夜,也不允許帶任何來賓、朋友、親戚于宿舍過夜;另外不得于宿舍內(nèi)吸煙、吸毒、喝酒;實(shí)習(xí)生應(yīng)尊重宿舍室友的個(gè)人隱私、健康、環(huán)境及舒適,并維護(hù)宿舍內(nèi)的清潔與衛(wèi)生。12. 實(shí)習(xí)生須自行負(fù)責(zé)個(gè)人疾病與看診費(fèi)用,因個(gè)人健康因素離開工作

21、期間不予支付實(shí)習(xí)薪資。實(shí)習(xí)單位負(fù)責(zé)實(shí)習(xí)生因工作而引起之相關(guān)醫(yī)療看診與住院費(fèi)用,醫(yī)療福利須完全依照實(shí)習(xí)單位之規(guī)定辦理。13. 實(shí)習(xí)生于新加坡實(shí)習(xí)期間, 由實(shí)習(xí)單位支付移民安全保證金5000 新加坡幣予新加坡政府,若實(shí)習(xí)生因任何犯罪而喪失實(shí)習(xí)機(jī)會(huì),須完全賠償實(shí)習(xí)單位無或 EWM Consultancy Pte Ltd 之移民安全保證金5000 新加坡幣。另外,實(shí)習(xí)生護(hù)照將由實(shí)習(xí)單位統(tǒng)一保管以免遺失。14. 實(shí)習(xí)生保證完全遵守新加坡法律以及實(shí)習(xí)單位與 EWM Consultancy Pte Ltd之相關(guān)規(guī)定。15. 實(shí)習(xí)生須承諾遵守 EWM Consultancy Pte Ltd 的相關(guān)規(guī)定。EWM

22、 ConsultancyPte Ltd、實(shí)習(xí)單位或同事等不負(fù)責(zé)個(gè)人的受傷、死亡、所有的花費(fèi)、遺失、賠償與支出。16. 前述 15 條款依新加坡法律管轄執(zhí)行之。除了上開建教合作實(shí)習(xí)代表約定事項(xiàng)外, 實(shí)習(xí)生于實(shí)習(xí)期間愿意配合學(xué)校督導(dǎo), 遵守貴校各項(xiàng)實(shí)習(xí)規(guī)章及生活條約,服從貴系實(shí)習(xí)委員會(huì)及實(shí)習(xí)單位之指導(dǎo)訓(xùn)練,為規(guī)范學(xué)校與簽署人間之權(quán)益,簽署人同意下列條款并遵守履行之。1.實(shí)習(xí)生于校外實(shí)習(xí)期間,仍為學(xué)校學(xué)生,優(yōu)良或不良表現(xiàn)依明新科技大學(xué)學(xué)生獎(jiǎng)懲辦法與旅館事業(yè)管理系校外實(shí)習(xí)學(xué)生獎(jiǎng)懲辦法辦理。2.實(shí)習(xí)生前往實(shí)習(xí)機(jī)構(gòu)實(shí)習(xí),依實(shí)習(xí)單位請(qǐng)假規(guī)定及勞動(dòng)法辦理,若實(shí)習(xí)單位無明確規(guī)定則依學(xué)校請(qǐng)假規(guī)定辦理。 海外實(shí)習(xí)生

23、應(yīng)完成一年海外旅行平安險(xiǎn)(附約疾病醫(yī)療住院險(xiǎn))事宜,并繳交保險(xiǎn)單據(jù)復(fù)印件予貴系后方赴海外實(shí)習(xí)。3.實(shí)習(xí)生于校外實(shí)習(xí)期間,未經(jīng)告知無故離職、工作懈怠或連續(xù)犯錯(cuò)等重大違規(guī)事件,經(jīng)貴系實(shí)習(xí)委員會(huì)派員調(diào)查,會(huì)同實(shí)習(xí)單位確認(rèn),經(jīng)本會(huì)通過移送本校學(xué)務(wù)處生活輔導(dǎo)組簽請(qǐng)學(xué)生獎(jiǎng)懲委員會(huì)處理,視情節(jié)得處以下列處分:(1)勒令退學(xué)。(2)記過處分,經(jīng)貴系實(shí)習(xí)委員會(huì)同意并由簽署人具結(jié)后,始得繼續(xù)實(shí)習(xí)。(3)勒令休學(xué),其已實(shí)習(xí)時(shí)數(shù)不予計(jì)算。簽署人實(shí)習(xí)生姓名:簽章:身份證字號(hào):護(hù)照號(hào)碼:生日:家長姓名:簽章:身份證字號(hào):手機(jī)號(hào)碼:住址:中華民國年月日無SPONSORSHIPAGREEMENT HOTEL12 MONTHS

27、odation. Dutymeal is provided free of charge by attachment employer.8. Daily transport and 2 ways air ticket to/from Singapore shall be Interns own cost.9. Either Party may terminate this Sponsorship Agreement by giving one (1) months notice in writing orpayment of 1 months allowance in lieu. The em

28、ployer is entitled to terminate the Sponsorship Agreementwithout advance notice and repatriate the Intern at Interns own cost in case of misconduct, poor performance,breaches laws, regulations or fraternization with guests or companys clients during or after work.10.Working schedule is strictly acco

29、rding to employers schedules. Intern may be attached to same section ordepartment for the complete 12 months at the Employers sole discretion. It is illegal for Intern to work withany other employer whilst in Singapore and illegal to take part in any drugs, alcohol, illegal, immoral,dishonest, crimi

30、nal or political activities while in Singapore.11.Intern shall not stay at any accommodation other than the accommodation provided by the employer or EWMand not allowed to bring any visitors, friends or relatives to visit or stay at the accommodation. Smoking,drugs, and alcohol consumption is prohib

31、ited in the accommodation.Intern shall respect the privacy, health,environment and comfort of others living in the accommodation. Intern is required to undertake cleaning andmaintaining the accommodation in clean & hygienic condition at all times.12.Intern shall be responsible for own illness and ou

32、tpatient medical treatment. Training Allowance is notpayable during medical leave. The employer is responsible for work related medical treatment andhospitalization only if it is caused directly from the employment with them. Medical benefits shall be solely inaccordance with employers policy.13.Intern shall fully reimburse the employer or EWM the Immigration Security Bond of S
