3、用身份證上的姓名/Must use the name on ID入職手續(xù)/On board procedure3. 合格的體檢報(bào)告原件/Original physical check report指定醫(yī)院體檢/Defined hospital某項(xiàng)指標(biāo)不合格必須復(fù)檢,合格后方能錄用/Have to re-check if not applicable4.無不良記錄證明 原件/Original non-criminal record戶口所在地派出所開具/issued by the police station where the huko located.一個(gè)月內(nèi)提供/Provide in one

4、 month5.暫住證復(fù)印件/Temporary resident permit暫住地派出所開具/issued by the police station where you currently living一個(gè)月內(nèi)提供/Provide in one month提供有效期內(nèi)的暫住證/provide valid permit入職手續(xù)/On board procedure需提前申請Apply in advance病假必須出具縣級以上醫(yī)院開具的證明Must submit hospital reference for Sick leave 請假至少半天,不可按小時(shí)請假At least half day

5、 for each application, can not take leave hourly請假制度/Leave application procedure書面申請并得到批準(zhǔn):提前30天,或以相應(yīng)工資替代離職通知期/ 30 days in advance, or by deducting relevant salary for substitute填寫辭職申請表/Written application and be approved: Resignation application辭職原因如實(shí)填寫/Clarify the true reason of leaving辦理手續(xù):離職通知書/

6、Resignation Notice工作交接、物品歸還/Return related work, equipments工資打入工資卡/Salary saved in card否則,本人持身份證原件到公司辦手續(xù)、領(lǐng)現(xiàn)金工資,不得代領(lǐng) Otherwise, go to company in person for related procedures, get salary. 離職手續(xù)/ Dismiss Procedure薪資/Salary每月15日,工資發(fā)放,5天后發(fā)放工資單/day15th of each month for salary receive, issuing the notice

7、5 days afterwards.工資卡號由員工向公司提供; 如有遺失、變動(dòng),必須及時(shí)通知人事部/Each employee will provide the Number of salary card ; Any loss, or destroy of the card, HR must be informed by employees on time.工資計(jì)算/Salary calculation依據(jù):考勤表/ Attendance sheet; 考勤周期: 1日31日/Attendance period: d1 - d31解決方案/Solution查詢/Enquiry有錯(cuò)必糾,多退少補(bǔ)

8、/correction下月調(diào)整/Adjust in next month工齡工資 - 項(xiàng)目點(diǎn)員工工作每滿1年,薪資增加50-100元,以此類推The basic employee salary for length of service - One year service, monthly salary will be added RMB 50-100, more and so on.入職&離職,發(fā)薪日不變/On-board & Dismiss, salary day is the same工傷、醫(yī)療、人身意外保險(xiǎn)/Injury, Medicare, accident insurances保

9、險(xiǎn)費(fèi):公司承擔(dān)工傷、醫(yī)療保險(xiǎn)和意外傷害保險(xiǎn)/ Compensation fee: company pays for injury, medical and accident insurances.出險(xiǎn)必須到縣級以上醫(yī)院就診,否則,不予理賠/In case of injury or accident, have to go to hospitals of county lever or above, otherwise, no compensation必須使用醫(yī)保范圍內(nèi)藥品,否則,不予理賠/To take pharmacies accepted by medical insurance, oth

10、erwise, no compensation.及時(shí)通知人事部/Inform HR on time.保存好所有單據(jù)(理賠須提供):/Store well the documents.病歷原件、發(fā)票原件/Medical bills, invoices. 住院清單、出院證明原件,如住院/Recovery references. etc. 如有骨折等傷殘情況,還須提供醫(yī)院證明/ Hospital references事情經(jīng)過說明,包括時(shí)間、地點(diǎn)、意外發(fā)生、搶救措施等/Description of the accidents離職,保險(xiǎn)自動(dòng)終止,公司不再負(fù)責(zé)理賠/Insurances suspended

11、 as employee dismissed from company.收費(fèi)問題/Fees公司收費(fèi)¥200 元,用于使用工服確認(rèn);Company charge 200 Yuan to affirm to using uniforms:200 元作為使用服裝確認(rèn),離職時(shí)歸還工作服,退還費(fèi)用;RMB200 for uniform, give back this fee after uniform returned 工作鞋根據(jù)使用期限確認(rèn)剩余價(jià)值,由員工認(rèn)購/The employee will buy the boot as the remaining cost.任何員工不得收取任何費(fèi)用/Any other employee cannot charge any fees.錄用權(quán)、分配權(quán)歸人事部,原則上,親戚朋友不安排在同一營運(yùn)點(diǎn)上/HR is the only dept. for hiring, assigning; generally, relatives will not be assign to same project.
