基于足底壓力分析足三陰經(jīng)筋深部刺激對中風后下肢運動特征的影響摘要:

目的:探討基于足底壓力分析足三陰經(jīng)筋深部刺激對中風后下肢運動特征的影響。

方法:選取中風后康復期下肢功能障礙患者30例,隨機分為實驗組和對照組,每組各15例。實驗組在康復訓練的基礎上加入足三陰經(jīng)筋深部刺激,經(jīng)過8周治療后,分別記錄兩組患者的足底壓力分布數(shù)據(jù),并進行分析。

結(jié)果:實驗組患者的足底壓力分布明顯優(yōu)于對照組,運動特征也發(fā)生了變化,如步態(tài)過程中踩地時間延長、支撐相對時間增加、步幅減小等。而對照組變化較小。

結(jié)論:基于足底壓力分析的足三陰經(jīng)筋深部刺激對中風后康復期下肢運動特征具有顯著影響,其中足底壓力分布的改善可能是致使步態(tài)改變的主要原因,從而提高下肢肌肉控制力和平衡能力,促進患者下肢康復。

Abstract:

Objective:ToexploretheeffectofdeepstimulationofFootShaoYinJingluobasedonplantarpressureanalysisonlowerlimbmovementcharacteristicsinpatientswithlowerlimbdysfunctionafterstroke.

Methods:Thirtypatientswithlowerlimbdysfunctionintherecoverystageafterstrokewereselectedandrandomlydividedintoexperimentalgroupandcontrolgroup,with15casesineachgroup.TheexperimentalgroupwasgivendeepstimulationofFootShaoYinJingluoonthebasisofrehabilitationtraining.After8weeksoftreatment,theplantarpressuredistributiondataofthetwogroupsofpatientswererecordedandanalyzed.

Results:Theplantarpressuredistributionoftheexperimentalgroupwassignificantlybetterthanthatofthecontrolgroup,andthemovementcharacteristicsalsochanged,suchaslongergroundcontacttime,increasedsupportrelativetime,andreducedstridelengthduringgaitprocess.Thechangewasrelativelysmallinthecontrolgroup.

Conclusion:ThedeepstimulationofFootShaoYinJingluobasedonplantarpressureanalysishasasignificanteffectonlowerlimbmovementcharacteristicsinpatientswithlowerlimbdysfunctionintherecoverystageafterstroke,amongwhichtheimprovementofplantarpressuredistributionmaybethemainreasonforthechangeofgait,anditcanimprovelowerlimbmusclecontrolandbalanceability,andpromotethelowerlimbrehabilitationofpatients.

SeveralstudieshaveinvestigatedtheeffectofFootShaoYinJingluostimulationonlowerlimbfunctioninstrokepatients.ArecentstudyconductedbyZhangetal.(2018)evaluatedtheeffectofFootShaoYinJingluoacupunctureandmoxibustiononthegaitofpatientswithlowerlimbdysfunctionintherehabilitationstageafterstroke.ThestudyfoundthatFootShaoYinJingluostimulationhadasignificanteffectonimprovinglowerlimbmovementcharacteristics,whichwasmainlyattributedtotheimprovementofplantarpressuredistribution.

Plantarpressureanalysisisareliablemethodforevaluatingfootfunction,andithasbeenwidelyusedinlowerlimbrehabilitation.Studieshaveshownthatstrokepatientshaveabnormalplantarpressuredistribution,whichaffectstheirgaitandbalance.BystimulatingtheFootShaoYinJingluo,thestudyfoundthatplantarpressuredistributionwassignificantlyimproved,whichinturnimprovedgaitandbalance.

Furthermore,theFootShaoYinJingluostimulationwasalsofoundtoimprovelowerlimbmusclecontrolandbalanceabilityinstrokepatients.ThisisconsistentwithTCMtheories,whichsuggestthattheFootShaoYinJingluoiscloselyrelatedtothemotorfunctionofthelowerlimb.

Inconclusion,FootShaoYinJingluostimulationhasasignificanteffectonimprovinglowerlimbfunctioninstrokepatientsintherehabilitationstage.Theimprovementofplantarpressuredistributionmaybethemainreasonforthechangeofgait,anditcanimprovelowerlimbmusclecontrolandbalanceability,promotingthelowerlimbrehabilitationofpatients.FurtherstudiesareneededtoexplorethemechanismofFootShaoYinJingluostimulationonlowerlimbrehabilitationinstrokepatientsStrokeisamajorcauseofdisabilityworldwide,andthelowerlimbiscommonlyaffected.Lowerlimbdysfunctionafterstrokecanseverelyimpairtheabilitytowalk,leadingtodecreasedmobilityandanincreasedriskoffalls.Rehabilitationisimportantforimprovinglowerlimbfunctionandpromotingrecoveryinstrokepatients.

AcupunctureisatraditionalChinesemedicinepracticethathasbeenusedtotreatvariousconditions,includingstroke.FootShaoYinJingluostimulation,atypeofacupuncture,involvesneedlingspecificacupuncturepointsalongtheShaoYinmeridianonthefoot.Thistechniquehasbeenusedtotreatlowerlimbdysfunctioninstrokepatients,butitseffectivenessandunderlyingmechanismsarenotwellunderstood.

SeveralstudieshaveinvestigatedtheeffectsofFootShaoYinJingluostimulationonlowerlimbfunctioninstrokepatients.OnestudyfoundthatFootShaoYinJingluostimulationcombinedwithconventionalrehabilitationsignificantlyimprovedlowerlimbfunction,includinggaitvelocityandstridelength,comparedtoconventionalrehabilitationalone.AnotherstudyfoundthatFootShaoYinJingluostimulationimprovedplantarpressuredistributionduringgait,suggestingthatitmayimprovelowerlimbmusclecontrolandbalanceability.

TheexactmechanismsunderlyingtheeffectsofFootShaoYinJingluostimulationonlowerlimbfunctioninstrokepatientsarenotclear.However,sometheoriessuggestthatacupuncturemaystimulatethenervoussystemandmodulatetheactivityofneurotransmitters,leadingtochangesinmusclefunctionandgait.Additionally,acupuncturemaypromotebloodflowandtissuehealing,whichcouldcontributetoimprovementsinlowerlimbfunction.

Inconclusion,FootShaoYinJingluostimulationappearstohaveasignificanteffectonimprovinglowerlimbfunctioninstrokepatientsintherehabilitationstage.Theimprovementofplantarpressuredistributionmaybethemainreasonforthechangeofgait,anditcanimprovelowerlimbmusclecontrolandbalanceability,promotingthelowerlimbrehabilitationofpatients.FurtherstudiesareneededtoexplorethemechanismofFootShaoYinJingluostimulationonlowerlimbrehabilitationinstrokepatientsInadditiontophysicalrehabilitation,traditionalChinesemedicine(TCM)hasbeenusedasacomplementarytherapyinthemanagementofstrokepatients.Jingluostimulation,whichincludesacupunctureandmoxibustiontherapy,iscommonlyusedtotreatstrokepatientsinTCM.

Acupunctureinvolvestheinsertionoffineneedlesintospecificpointsonthebodytoreliefpainandtreatvariousconditions.Moxibustionisaheattherapythatinvolvestheburningofdriedmugwort,alsoknownasmoxa,nearorontheskintostimulatetheacupuncturepoints.Jingluostimulationworksbyregulatingthecirculationofqiandbloodinthebodyandpromotingthebody'snaturalhealingprocesses.

SeveralclinicalstudieshavereportedtheeffectivenessofJingluostimulationinthemanagementofstrokepatients.Ameta-analysisofrandomizedcontrolledtrials(RCTs)foundthatacupuncturetherapyhadasignificanteffectonimprovingmotorfunction,walkingability,andactivitiesofdailylivinginstrokepatients(Luetal.,2015).AnotherRCTshowedthatmoxibustiontherapycombinedwithconventionalphysiotherapyimprovedthewalkingabilityandqualityoflifeinhemiplegicpatients(Yooetal.,2017).

AlthoughtheprecisemechanismofJingluostimulationinstrokepatientsisstillunclear,ithasbeenproposedthatitworksbyimprovingbloodflowtothea
