IgG4相關(guān)性腎病

IgG4-relatedkidneydisease(IgG4-RKD)BACKGROUNDIgG4-relateddiseasehasattractedwideattentionrecently.ItischaracterizedbyahighlevelofserumIgG4anddenseinfiltrationofIgG4-positiveplasmacellsintomultipleorgans,withthekidneybeingonerepresentativetarget.BACKGROUNDToclarifytheentityofIgG4-relatedkidneydisease(IgG4-RKD)andsupportin-depthstudies,theJapaneseSocietyofNephrologyhasestablishedaworkinggrouptopreparediagnosticcriteriaforIgG4-RKD.METHODTheworkinggroupanalyzed41patientswithIgG4-RKD,andcollectedthefollowingdatatodeviseadiagnosticalgorithmanddiagnosticcriteriaforIgG4-RKD:clinicalfeaturesincludingextra-renalorganinvolvement,urinalysisandserologicalfeaturesincludingserumIgG4levels,imagingfindingsdemonstratedbycomputedtomography(CT),renalhistologywithIgG4immunostaining,andresponsetosteroidtherapy.RESULTSTheconditionsforcriteriaareasfollows：

(1)Presenceofsomekidneydamage,asmanifestedbyabnormalurinalysisorurinemarker(s)and/ordecreasedkidneyfunctionwitheitherelevatedserumIgGlevel,hypocomplementemia,orelevatedserumIgElevel.診斷標(biāo)準(zhǔn)(2)Kidneyimagingstudiesshowingabnormalrenalimagingfindings,i.e.,multiplelowdensitylesionsonenhancedCT,diffusekidneyenlargement,hypovascularsolitarymassinthekidney,andhypertrophiclesionoftherenalpelvicwallwithoutirregularityoftherenalpelvicsurface.診斷標(biāo)準(zhǔn)(3)SerumIgG4levelexceeding135

mg/dl.診斷標(biāo)準(zhǔn)(4)Renalhistologyshowingtwoabnormalfindings:(a)denselymphoplasmacyticinfiltrationwithinfiltratingIgG4-positiveplasmacells>10/highpowerfield(HPF)and/orratioofIgG4-positiveplasmacells/IgGpositiveplasmacells>40%.(b)Characteristic'storiform'fibrosissurroundingnestsoflymphocytesand/orplasmacells.診斷標(biāo)準(zhǔn)(5)Extra-renalhistologyshowingdenselymphoplasmacyticinfiltrationwithinfiltratingIgG4-positiveplasmacells>10/HPFand/orratioofIgG4-positiveplasmacells/IgG-positiveplasmacells>40%.結(jié)果Thediagnosisisclassifiedinto3stagesofdefinite,probableandpossibleaccordingtothecombinationsoftheaboveconditions.Thirty-ninecases(95.1%)werediagnosedwithIgG4-RKDaccordingtothecriteria.CONCLUSIONTheprovisionalcriteriaandalgorithmappeartobeusefulforclarifyingtheentityofIg
