1、大型游樂設施驗收需辦理的相關法定程序Relevant Legal Procedures of Special Equipment Acceptance程序一:在簽訂設備合同前,賣方負責將所銷售設備的圖紙及技術資料提供給中國特種設備檢測研究院進行設計審核,經審核合格,并取得設計文件鑒定報告后,方可生產設備。Phrase I: Before signing the Contract, the Seller shall provide the drawings and technical information of the ride to CSEI for getting the “Design

11、gn Review Institution程序二:簽訂合同后,賣方負責向國家質檢總局特種設備許可辦公室申請首臺設備型式試驗備案。Phrase II: After the Contract signed, the Seller is responsible to apply the first Machine Type Test Record from Special Equipment Office of CSEI.進口機電產品(索道及游樂設施)型式試驗備案申請需遞交如下資料:Necessary documents for applying Type Test of imported Mech

12、anical and Electrical Machine (for Ropeway and Attractions)1、中文版特種設備制造許可申請書(編號:TSZS001-2003) 4份)及電子版文件(即磁盤)。1) “Application form for Special Equipment Manufacturing” (code: TSZS001-2003) 4 copies in Chinese version and E-Version ( CD-ROM)2.境外制造工廠簽署的“聲明”(中文),主要內容包括:境外制造工廠承諾在中國境內銷售的*產品遵守中國現(xiàn)行法規(guī)和安全技術規(guī)范,

13、并承擔因違反中國現(xiàn)行法規(guī)、安全技術規(guī)范所造成的一切后果。境外制造工廠承諾為在中國境內使用*產品的用戶提供及時周到的支持和服務。境外制造工廠授權*公司在中國境內辦理型式試驗備案相關手續(xù),并承擔相關質量、安全和法律責任。2) The Statement signed by Overseas Manufacture, with contents as following:“The Overseas Manufacture commits thatits Ride products sold in China arein conformitywith Chinese regulations and s

14、afety technical specifications, and will take the consequences caused due to its violation of the regulations and specifications mentioned above.The Overseas Manufacture will provide our clients with good and timely technical support and service for amusement rides sold in China, and will take all t

15、he consequences caused by its unqualified technical support and service.The Overseas Manufacture hereby entrust (Authorized Company) to handle the formalities for type test record-filing, application for installation of type test ride sample, and will undertake the related safety, quality andlegalco

16、nsequences and responsibilities.”3.國內代理商營業(yè)執(zhí)照和組織機構代碼證的復印件;3) Copies of business license and organization code of domestic Agent4.境外制造廠的商業(yè)注冊文件和相關認證證書的復印件。4) Copies of Commerce registration documents and related Certificates of overseas Manufacture5.申請產品的樣本(圖片)5) Sample of applied product ( picture )6)

17、賣方授權給買方代表賣方辦理上述程序2,但是賣方將會提供買方所要求的必須資料。6) The seller entitles the Buyer to proceed with PHASE II on behalf of the Seller, but Seller shall provide necessary documents to Buyer at the request of the Buyer.程序三:賣方持已受理的特種設備制造許可申請書約請型式試驗機構進行型式試驗(填寫游樂設施整機(部件)型式試驗申請表)。Phrase III: With approved “Special Equi

18、pments Manufacture Application”, The Seller shall invites Type Test Institution fill the Equipment whole ride (Parts Type Test Application Form.程序四:賣方向江西省質監(jiān)局特種機電設備處申請安裝1臺型式試驗樣機。Phrase IV: The Seller applies to install the Type Equipment from the Special Mechanic and Electric Equipment Office of Guan

19、gzhou Quality Control Bureau. 申請樣機批復需提交資料Necessary documents for Type Test Approval1、型式試驗樣機試安裝申請報告1份1. “Type Equipment installation Application” one origin file2、已受理的特種設備制造許可申請書復印件1份2. Approved “Special Equipments Manufacture Application” one copy3、中國特種設備檢測研究院同意約請的游樂設施整機(部件)型式試驗申請表3. “Type Test Appl

20、ication Form for the Whole Amusement Ride (or parts)” approved by CSEI4、制造或銷售合同復印件1份4. A copy of Manufacture Contract or Sale Contract5、申請單位法人營業(yè)執(zhí)照復印件1份5. A copy of “corporate business license “of Applicant6、申請單位組織機構代碼證復印件1份6. A copy of “Organization Code” of Applicant7、特種設備安裝改造維修許可證復印件1份7. A copy of

21、 “Special Equipment Installation/Modification License” 8、特種設備安裝合同或安裝委托書1份(制造單位無安裝資格時)8. Origin of Special Equipment Installation Contract or Authorized letter of Installation (when the Manufacture Company doesnt have the Installation License)以上所有復印件需加蓋申請單位公章,申請時應同時提供原件核對確認。Applicant should apply Off

22、icial Stamp for all the copies of the above documents, meanwhile provide original documents for verification.程序五:憑省局開出的樣機批復函,賣方委托安裝公司到深圳市市場監(jiān)督管理局辦理安裝告知。Phrase V:The Seller entrusts Installation company to deal with Installation Approval with replied “Type Equipment Installation Application” from Shen

23、zhen Quality Supervision Bureau.安裝告知所需資料:Necessary documents for Installation Approval1、九江市特種設備安裝(改造、維修)告知表(原件1份);1. An origin of “ Shenzhen Special Equipments Installation ( Modification/Maintenance) Application”2、特種設備安裝改造維修許可證(加蓋單位公章的復印件1份,驗原件);2. A copy of “Special Equipment Installation/Modifica

24、tion License” (with company stamp and origin documents prepared)3、施工人員特種設備作業(yè)人員證(加蓋單位公章的復印件1份,驗原件);3. Copy of Construction Operators License of Special Equipments (with company stamp and origin documents prepared)4、使用單位機構代碼證復印件1份;4. A Copy of Users Organization code5、施工項目合同及日常維修保養(yǎng)合同(加蓋單位公章的復印件1份,驗原件)

25、;5. A copy of Construction Contract and daily Maintenance Contract (with company stamp and origin documents prepared)6、項目施工方案及其安全防護措施原件1份;6. An origin documents of Construction Plan and Safety Measurements7、國產設備:產品合格證(加蓋單位公章的復印件1份,驗原件); 如果特種設備產品合格證上注明的使用單位與告知的實際使用單位不同,應提供由上述雙方單位簽署的產品委托訂貨證明或轉讓證明(加蓋雙方

26、單位公章的復印件1份,驗原件)如制造廠的設備初次來九江或制造廠的制造許可信息發(fā)生變更,窗口受理人員可要求申報單位提供制造廠家加蓋單位公章的制造許可證、組織機構代碼證和營業(yè)執(zhí)照復印件各1份;7. For Domestic Equipments needs:A Copy of Product Quality Certificate ( with company stamp and and original documents prepared )If the User in the Product Quality Certificate is different from the User in

27、the Installation Approval, both Users shall provide the copy of signed Product Order Entrustment or Product transferring Documents (with both companys Stamp and origin documents prepared).If it is the first time that the Manufacture come to JIUJIANG or theres some change of the Manufacture License,

28、the officers can require the company to provide a copy of Manufacture License, Organization Code and Business License with company stamp.8、境外生產設備:大型游樂設施須提供型式試驗報告(或省局開出的樣機批復函)和大型游樂設施設計文件鑒定報告(原件各1份)。注:需進行設計文件鑒定的大型游樂設施,見大型游樂設施設計文件鑒定規(guī)則(試行)(國質檢鍋2003321號));8. For Overseas Manufacture:Origin documents of T

29、ype Test Report of Large-scale Special Equipments (or Installation Application Approval by Local Province Bureau) .Note: If the Design Review for Special Equipment is needed, please obey the “Special Equipments Design Review Rules (trial implement)” (Chinese Quality Control Bureau Documents No. 2003

30、321.9、如屬設備移裝的,還需提供下列材料:a設備在本市內移裝的,使用單位需提供移裝申請(申請內容應注明其移裝設備注冊號、原因及移裝前后設備安裝地點)、設備出廠資料、首次驗收檢驗報告和定期檢驗報告。使用單位發(fā)生變更的,須先辦理過戶手續(xù);b設備從市外移裝到本市的,使用單位除提供上述材料外,還應提供設備在原注冊地的注銷證明。9. If the Equipments need to be moved, following documents are needed.a. If the ride needs to move within the city, the User shall provide

31、 the Transferring Installation Application (including: Ride Registration No, Transferring Reason and New Installation place), Ride Manufacture documents, Inspection Report of first time, Regular Inspection Report. If the Equipment User changes, Product Hand-Over procedure is need to be done.b. If th

32、e rides moves into the city from other cite, the User shall provide all the above documents and Write-off information from origin registration place.程序六:賣方和安裝公司在安裝自檢合格(80小時無故障測試完成)后,買方配合邀請中國特種設備檢測研究院檢驗人員現(xiàn)場進行型式試驗和監(jiān)督檢驗,并取得特種設備型式試驗報告。Phrase VI: After the Seller and Installation Company finished Install

33、ation-Inspection (the 80 hours non-stop test qualified), the buyer shall cooperate to invite CSEI to do site Type Test and Ride Inspection and Supervision to get the “Special Equipments Type Test Report”.游樂設施監(jiān)督檢驗所需資料清單Required Document List for Amusement Rides Supervision & Inspection 1、制造隨機文件(1)裝箱單

34、或裝車單(新設備)(2)總圖、重要受力部件圖、維修保養(yǎng)必備的機械、電氣、液壓、氣動等部分圖紙及易損件圖紙 (3)產品質量證明文件,至少包括產品合格證、重要受力部件材質一覽表和材質證明書、重要焊縫和軸銷類的探傷報告、標準機電產品合格證、外委加工受力結構或受力部件的質量證明文件、制造檢驗記錄、硬度測定報告(所有記錄與報告應簽字齊全)。材料質量證明文件必須包括計算書計算項目的材料和其他受力結構和受力部件材料,材質單應清晰、完整、內容符合標準與設計計算要求。(4)使用維護說明書(5)制造許可證和安裝改造許可證書復印件 (6)安全技術規(guī)范規(guī)定的其他資料 1) Manufacturing Random D

35、ocument1) Packing List or Loading List (new ride)2) General Plan, Drawings for the Crucial Stressed Components, Drawings for the Parts such as Mechanical, Electrical, Hydraulic and Pneumatic etc Necessary for Maintenance and Drawings for Easily Damaged Parts3) Product Quality Certification, includin

36、g at least Product Quality Certificates, Material List of the Crucial Stressed Components and Material Certificates, NDT Test for Crucial Welding Seams and Shafts & Pins, Certificate of standard Mechanic and Electrical Machine, Quality Certificate of Stressed members that processed by Authorized com

37、pany, Manufacturing Inspection Record, Hardness Test Report (including all signed record and report). Material Quality Certificate should including Materials that used in stressed members or structures which appears in the Calculation Manual. Material List should be clear, complete and its standard

38、meets related standards and design calculation requirements.4) Operation and Maintenance Manual5) Copy of the Manufacturing License and Installation Modification License6) Other Relevant Documents as Required by Safety Specifications 2、設計審查文件鑒定報告、同類型首臺設備的型式試驗報告(約請函和樣機批復函)2) Design Review Approval Re

39、port and First ride Type-test Report (Invitation and Type Equipment Application Approval)3、配套土建基礎技術圖樣等有關技術文件資料和有資質單位的基礎驗收報告 3) Relevant Technical Documents (e.g. Supporting Civil Work Technology Pattern) and Inspection Report on the Foundation Constructed by a Qualified Company4、安裝合格證書(有簽發(fā)權的人員簽字和安裝單

40、位公章)4) Installation Quality Certificate (with Authorized staffs signature and Installation company stamp5、設計變更表:設計變更表應有制造單位技術負責人簽字,應包括主要材料的牌號與規(guī)格變更、結構變更、標準機電產品選型變更等。5) Design Alternation Form with Technical Principals Signature of Manufacturer which should include alternations on the main material gr

41、ades, specifications, structures and types of standard mechanical and electric Products etc.6、安裝/改造方案、工藝(包括重要部位與部件的焊接與熱處理)、施工記錄;安裝/改造自檢記錄與檢驗合格報告 6) Installation/Modification Proposals, Workmanship (including welding and heat treatment for the crucial positions and components) and Construction Record

42、; Self-check Report on Installation/Modification and Qualified Inspection Report7、現(xiàn)場安裝/改造所用主要受力部件材質證明 7) Material Certificates for the Main Stressed Components used for On-site Installation/Modification8、制造、安裝或改造無損探傷報告 8) NDT Test for Manufacturing, Installation or Modification9、安裝或改造施工告知書9) Install

43、ation or Construction Modification Notification10、運轉試驗記錄,應按次、按時記載,應有設計規(guī)定的各類運行參數(shù)實測值和試驗載荷實測值,還應有問題或故障處理的詳細記載。監(jiān)督檢驗時,安裝單位至少應運行足夠時間并記錄載荷試驗情況。10) Running TestTest Run Record, which recorded according with time and frequency, different designed operational parameters and Capacities tested onsite, defaults

During supervision and inspection period, the installation company shall provide enough Test Run record and Capacity situation.11、產品主要材料選用情況核實表(設計材料和實際使用材料做比對)11) Product Material check list ( A comparison between designed materials and applied materials)12、安裝或改造施工告知書12) Installation

45、Notification or Construction Modification Notification13、特種設備作業(yè)人員證書13) Special Equipment Operator Certificate14、乘客須知14) Ride Instructions15、緊急救援預案15) Emergency Rescue Plan16、原登記機關注銷使用登記注冊手續(xù)(易地安裝時)16) Process of Registration Cancellation by the Original Registration Authority ( in case of changing in

46、stallation site)17、產品銘牌,內容至少應包括制造單位名稱與地址、設備名稱、編號、生產許可證號以及速度、高度、額定乘客數(shù)等技術參數(shù)。17) Nameplate which at least including Manufacture Company name, Address, Ride Name, Ride Number, Manufacture License Number and Ride Speed, Height, Capacity and Technical Parameters.程序七:型式試驗通過后,由型式試驗機構向許可辦遞交型式試驗報告,報質檢總局審批后予以備

47、案。Phrase VII. After the Type Test approved, the Type Test Organization shall hand over the “Type Test Report” to Special Equipment Office, Which the Quality Control Bureau will keep all these information on record.程序八:備案完成,中國特種設備檢測研究院簽發(fā)特種設備監(jiān)督檢驗報告,設備可投入使用。Phrase VIII: The CSEI will issue “Special Equ

48、ipments Supervision and Inspection Report” after document record, only at the moment, the ride can be opened to the public.程序九(買方辦理):在投入使用1個月內,向深圳市市場監(jiān)督管理局申請使用注冊登記。Phrase IX : The Buyer shall apply to register the ride from Shenzhen Local Quality Control Bureau one month before opening.申請使用注冊登記需提供如下資
