3、SECCO乙烯裝置急冷區(qū)設(shè)備， 1號急冷水冷卻器(E-1501A/B)，外形尺寸為2070（外徑）×11088（全長），設(shè)備凈重80噸；2號急冷水冷卻器(E-1502A/B)，外形尺寸為1943（外徑）×13650（全長），設(shè)備凈重77.7噸。這四臺設(shè)備均為臥式設(shè)備，E-1501A/B位于No.1鋼結(jié)構(gòu)框架平臺上（鞍座標(biāo)高Ñ9725），(E-1502A/B)位于混凝土基礎(chǔ)上（基礎(chǔ)標(biāo)高Ñ2280），擬采用單機(jī)吊裝的吊裝工藝進(jìn)行安裝，使用LR1280型履帶式吊車（280t）吊裝，為保證設(shè)備吊裝的順利進(jìn)行及安全，特編制本方案。In quench area of

22、nagement measures7.1施工過程中可能發(fā)生的安全事故風(fēng)險分析 JHA during construction施工現(xiàn)場地質(zhì)條件差，易發(fā)生吊車傾覆事故；Bad ground condition on site will easily cause crane overturn;設(shè)備吊裝施工屬于高空作業(yè)，易發(fā)生高空墜落事故；Equipment lifting needs work at heights, and falling accident is easily occurred;物體打擊；Stricken by objects;設(shè)備運輸事故；Transportation accid

23、ent;鋼絲繩、卡具不合格；Unqualified wire rope and shackles;吊裝作業(yè)過程中，發(fā)生吊裝物件傾倒，損壞吊車等；Equipment overturns and crane damage during lifting;在平臺和坑、洞口旁作業(yè)或路過時發(fā)生墜落事故；Falling accident from platform, pit, hole during working or passing by;吊耳和平衡梁制作、焊接不合格，發(fā)生墜落事故。Unqualified lifting lug and compensating beam cause falling a

24、ccident;7.2相應(yīng)對策 Corresponding measures加強(qiáng)安全教育，嚴(yán)格執(zhí)行班前安全講話和周一安全活動制度，有針對性地交代近期工作中的安全注意事項；Enhance safety training, strictly carry out the system of tool box meeting and routine Monday safety meeting, make safety clarification according to recent work;凡參加大型設(shè)備吊裝的工作人員，如安全監(jiān)督、吊裝指揮、配合吊裝作業(yè)的起重工人、吊車操作員等，必須持有國家有關(guān)部

25、門頒發(fā)的上崗證。設(shè)備吊裝前，對所有參加吊裝的作業(yè)人員進(jìn)行安全和技術(shù)交底。All participated personnel such as safety supervisor, lifting commander, rigger and crane operator etc. shall have qualification certificate issued by related national authority, tell all persons take part in lifting real intentions about safe and technical parame

26、ters.針對物體打擊和高空墜落事故的原因，采取下列措施：Following measures shall be taken to avoid object striking and falling from height:1）戴好安全帽，系好安全帶，高空作業(yè)時做到安全帶高掛低用，使用安全帶前仔細(xì)檢查安全帶有無損壞。高處作業(yè)、交叉作業(yè)應(yīng)張掛安全網(wǎng)，有高處作業(yè)處下方12米必須有安全網(wǎng)，否則不得施工。Correctly use safety helmet and harness. Harness shall be hung at height and used at lower level, an

Safety net shall be used during work at height and cross work, and shall be provided 12m under work position, otherwise, work is forbidden.2）高處作業(yè)應(yīng)使用合格的腳手桿、吊架、梯子、腳手板，防護(hù)圍欄，擋腳板和安全帶，作業(yè)前應(yīng)認(rèn)真檢查所用安全設(shè)施是否堅固、牢靠，腳手架應(yīng)經(jīng)安全員檢查認(rèn)可后，掛牌標(biāo)識，并辦理工序交接手續(xù)后，方可使用。Qualified scaffold pole, hang

29、不易失落處，防止砸傷下方人。Tools and other materials shall be put at safe place during work at height to avoid falling and striking accident.4）為防止設(shè)備運輸事故的發(fā)生，必須認(rèn)真封車，運前探路，小心謹(jǐn)慎，確保安全。To ensure safety and avoid transport accidents, equipment must be fixed carefully and transport route shall be ascertained 5）吊裝作業(yè)區(qū)應(yīng)設(shè)置明顯

30、的警告牌，并設(shè)專職安全員負(fù)責(zé)安全監(jiān)護(hù)。作業(yè)前，將可能發(fā)生事故的區(qū)域隔離，無關(guān)人員不得入內(nèi)；Warning board shall be set up at eye-catching position in lifting area, and full-time safety inspector shall be arranged for supervision. Prior to lifting, potential accident area must be isolated and closed to irrelevant person.6）為防止發(fā)生吊裝事故，手拉葫蘆使用前應(yīng)檢查是否完好

31、，不得超載使用，嚴(yán)禁強(qiáng)拉硬拽；檢查鋼絲繩、卡具是否合格。吊裝作業(yè)時，人員不得站在重物正下方，吊裝指揮穿著由SECCO公司指定的指揮人員服裝進(jìn)行指揮，指揮信號應(yīng)統(tǒng)一、明確。安裝的設(shè)備應(yīng)及時固定，確保穩(wěn)固性。To avoid lifting accident, chain block shall be checked before using, and overload and over-tension shall be forbidden. Check if wire rope and shackle are qualified. During lifting personnel shall n

32、ot stay below the lifted equipment, the commander shall wear the uniform designated by SECCO. The equipment shall be fixed timely.7）在坑、洞、平臺邊緣等“四口五臨邊”處設(shè)置護(hù)欄，防止高空墜落事故。Guardrail shall be set up at the edges of pit, hole and platform to avoid falling from height.8）施工場地是由填海形成的，遇到陰雨天氣易積水，因此應(yīng)作好場地的排水措施。陰雨天氣不

33、進(jìn)行吊裝作業(yè)，雨天過后，應(yīng)對場地進(jìn)行檢查，確認(rèn)條件具備后，才能進(jìn)行吊裝作業(yè)。Construction site is formed by sea reclamation, and easily accumulate water in rainy days, so dewatering measures shall be taken. Lifting work shall be stopped in rainy days, and can be proceeded only after the ground condition is acceptable.9）針對每一步工序作業(yè)前，組織由總指揮、

34、工程技術(shù)部、質(zhì)量部門、安全部門、供應(yīng)部門、施工班組等聯(lián)合參加的吊裝JHA分析專題會議，制定合理、有效的保障措施。如發(fā)生安全事故，應(yīng)及時搶救受傷人員，保護(hù)現(xiàn)場，并對事故原因加以分析，防止同類事故再次發(fā)生。According to the characteristic of each working procedure, organize the lifting JHA meeting participated by chief commander, relative personnel from engineering dept., QA dept., HSE dept., procuremen

If accident occurs, save the injuries timely, protect the accident site, and analyze accident reasons to avoid same accident.10）吊車站位處地面的地耐力應(yīng)滿足吊裝工藝要求，吊裝過程中應(yīng)隨時檢查吊車支腿處地基的變化情況，如發(fā)現(xiàn)異常，應(yīng)立即停止吊裝作業(yè)。The bearing capacity of

41、周邊的雜物清理干凈，防止在吊運過程中雜物掉落傷人。13設(shè)備吊裝綁扎方法不正確導(dǎo)致吊物傾斜。SPR按照方案要求正確綁扎，并畫出綁扎點的位置圖。1224起重工綁扎鋼絲繩作業(yè)時傷手。133戴好防護(hù)手套，注意與吊車指揮人員的協(xié)調(diào)配合。鋼結(jié)構(gòu)移動時碰傷作業(yè)人員或其他人員。2241、專人指揮，無關(guān)人員不得進(jìn)入作業(yè)區(qū)；2、設(shè)備移動時用溜繩或搭鉤進(jìn)行輔助移動、固定。惡劣的天氣或惡劣的工作條件使作業(yè)風(fēng)險增大。224禁止在夜間,大雨，雷電，風(fēng)速大于10.8米/秒的情況下進(jìn)行吊裝作業(yè)。設(shè)備在就位時擠傷鉚工或起重工的手。2361、 設(shè)備就位時用溜繩或搭鉤進(jìn)行輔助固定。2、吊車指揮人員要看清情況后進(jìn)行指揮，設(shè)備就位時要

46、veracity and integrity.8.6施工所用計量器具要定期送檢，確保準(zhǔn)確有效。Measurement apparatus shall be calibrated termly to ensure accuracy.8.7吊裝作業(yè)時，人員不得站在重物正下方，指揮信號應(yīng)統(tǒng)一、明確。作業(yè)前，將可能發(fā)生事故的區(qū)域隔離，設(shè)立吊裝作業(yè)專用區(qū)，設(shè)圍欄，與吊裝工作無關(guān)人員嚴(yán)禁入內(nèi)，專職安全員負(fù)責(zé)現(xiàn)場安全工作。During lifting, personnel shall not stay below the lifted equipment; the command shall be unif
