INCIDENT DESCRIPTION2011年8月29日，早上9：00點(diǎn)多，承包商主管余智福先生到消防樓改造現(xiàn)場檢查，發(fā)現(xiàn)有些工人沒有佩戴安全眼鏡, 也沒有看到寶鷹現(xiàn)場負(fù)責(zé)人曾秋華和安全員于洪成，馬上打電話要求他們立即來到現(xiàn)場。他們答復(fù)半小時回來但直到12點(diǎn)多才來到現(xiàn)場。Shortly after 9:00am on August 29, the contracto

3、r supervisor, Mr. Yu Zhifu checked fire building modification site and found some Bao Ying workers dont wear safety glasses as well as the site responsible people, Mr. Zeng and Mr. Yu Hongcheng arent at present, he immediately called them and requested them to terminal as soon as possible, they prom

4、ised to come back to terminal within half hour but they showed up in terminal after 12:00am. 大約10：00點(diǎn)時候，倒班監(jiān)督馮道榮發(fā)現(xiàn)寶鷹公司工人在施工，有些工人沒有佩戴合適的PPE，也沒有作業(yè)許可證，立即停止他們的工作，隨后向接收站經(jīng)理匯報(bào)，通知了安全工程師和大鵬公司工作聯(lián)系人何金根。 About 10:00am, shift supervisor Mr. Feng Daorong found Bao Ying workers working without permit and some worke

5、rs not wearing proper PPEs, so he stopped their work immediately and reported to terminal supervisor, notified safety engineer and GDLNG contact person, Mr. He Jingen. 調(diào)查發(fā)現(xiàn)，8月28日從下午3點(diǎn)直到下班，寶鷹公司現(xiàn)場負(fù)責(zé)人曾秋華和安全員于洪成都沒有在接收站，而工作仍然進(jìn)行，也沒有歸還工單。It was found that the site responsible people, Mr. Zeng and Mr. Yu Ho

6、ngcheng werent in terminal while work still ongoing in modification site from 3:00pm to off time on August 28, the permit wasnt returned. 8月29日上午8點(diǎn)，倒班監(jiān)督馮道榮因?yàn)閷汑椆?8號沒有及時歸還許可證，且臨時派來申請?jiān)S可證的人沒有資格，拒絕簽發(fā)寶鷹公司的作業(yè)許可證。隨后大鵬公司工作聯(lián)系人何金根與馮道榮進(jìn)行了協(xié)商，電話要求寶鷹公司現(xiàn)場負(fù)責(zé)人盡快趕回接收站。 About 8:00am on August 29, shift supervisor, Mr.

7、 Feng Daorong refused to issue permit to Bao Ying company due to they didnt returned permit in time on August 28 plus the temporary permit applicant didnt receive PTW training. Afterwards he discussed with GDLNG contact person, Mr. He Jingen, made phone call to urge Bao Ying site responsible people

8、to come back to terminal as soon as possible. 目前，寶鷹公司仍然在停工等待處理。Until now, fire building modification work is still on suspension. 4.
