1、.WORD.格式整理.1 目的 /Purpose規(guī)范廠區(qū)內(nèi)危險作業(yè)申請操作流程與作業(yè)規(guī)范。In order to enhance the man ageme nt of hazardous operati on, avoid happe ning accide nts, protect people in safety, we formulate this regulati on.2. 范圍 /Scope適用于全公司范圍內(nèi)的危險作業(yè)安全管理，包括動火作業(yè)、高處作業(yè)。This procedure ruled all hazardous operati ons in clud ing hot wo

2、rk; work at height;3. 定義/Defi niti ons3.1 動火作業(yè)：進(jìn)行焊接與切割作業(yè)及在易燃易爆場所使用噴燈、電鉆、砂輪等進(jìn)行可能產(chǎn)生火焰、火花和赤熱 表面的臨時性作業(yè)；Hot work : Any work involving welding, cutting, grinding or other similar operation produc ing sparks3.2 高處作業(yè)：利用腳手架、升降平臺、便攜式木梯和金屬梯等登高工具從事超過基準(zhǔn)面2米以上，存在墜落風(fēng)險的作業(yè)；Work at height:4. 程序 /Procedure序號NO.危險作業(yè)內(nèi)容Ha

3、zardous operati on4.1動火作業(yè)Hot work4.1.1職責(zé) Resp on sibility4.1.1.1申請人的職責(zé)（必須由我司項目負(fù)責(zé)人擔(dān)當(dāng)）：1.動火作業(yè)前負(fù)責(zé)動火作業(yè)申請，并遵守“一天一證一動火點”為原則；2對外來施工單位的資格合法性審查，提醒外來施工單位帶齊動火所需要的相關(guān)證件、防護(hù)裝備；以備 我司檢查；3. 對動火作業(yè)現(xiàn)場進(jìn)行確認(rèn)，確?，F(xiàn)場符合動火要求；4. 對動火人員的勞動防護(hù)、動火設(shè)備的安全狀態(tài)負(fù)責(zé)；5. 對動火人員進(jìn)行培訓(xùn)，確保熟悉動火作業(yè)要求、消防器材使用方法和應(yīng)急處理流程（必 要時，聯(lián)系EHS人員助指導(dǎo)）；6. 每天上、下午各不少于一次不定時對動火現(xiàn)

4、場進(jìn)行巡視，發(fā)現(xiàn)異常立即予以糾正；7. 所有供應(yīng)商外服人員、外來施工人員等進(jìn)行的焊接、氣割、烘烤等明火作業(yè)均需要提前申請；Resp on sibility of applica nt （The applica nt should be the project leader）:1. Check out fire operator1 s PPE and tools5 Train fire operators about safety requireme nts for hot work, and how to use fire facilities and evacuati on .(If n ecessary, con tact EHS)6. Check out the

7、 job site at least one time irregular in morning and after noon. Shut down and make corrective action if there are any unsafe conditions or acti ons regard ing cutt ing, weldi ng and/ or heati ng operati ons.7. Go to job site and check out the fire-proof measures before approving the operation, ensure that it1 s4.1.1.3動火人職責(zé)1. 查驗動火作業(yè)申請單審批情況，無動火作業(yè)申請單、申請單上相關(guān)流程未審批完畢的應(yīng)拒絕 進(jìn)行動火作業(yè)；2. 逐項確認(rèn)相關(guān)安全措施的落實情況；3. 確認(rèn)動火地點和時間，是否和

9、動火作業(yè)申請單一致；4. 發(fā)現(xiàn)不具備安全條件時不得進(jìn)行動火作業(yè)；5. 隨身攜帶動火作業(yè)申請單，以備檢查；6. 所有金屬焊接、氣割作業(yè)人員必須有“金屬焊接切割作業(yè)操作證”,并熟悉動火要求及消防器材使用 方法；7. 動火人員必須佩戴焊工專用絕緣手套、絕緣鞋及其它防護(hù)用品；8. 在動火作業(yè)完成后，清理現(xiàn)場，清除殘火，確認(rèn)無遺留火種后方可離開現(xiàn)場。Resp on sibility of fire operator:1 Check out hot work got approval. 11、site6. Fire operator must be trained specially and get the lice nse.7 Wear proper pers onal protective equipme nt such as gloves, safety shoes and face shield.8. Clean the job site; make sure no fire in job site when finishing fire operation.4.1.1.4監(jiān)火人職責(zé)1.堅守崗位，不準(zhǔn)脫崗若暫時離開需找適合人員替崗，并盡快返回；動火期間，不準(zhǔn)兼做其它工作；2

12、. 負(fù)責(zé)現(xiàn)場動火前的準(zhǔn)備工作并監(jiān)護(hù)與檢杳，發(fā)現(xiàn)異常情況應(yīng)立即通知動火人停止動火作業(yè)，及時聯(lián)系有關(guān)人員采取措施；3. 當(dāng)發(fā)現(xiàn)動火人違章作業(yè)時應(yīng)立即制止；4. 在動火作業(yè)完成后，應(yīng)會同有關(guān)人員清理現(xiàn)場，清除殘火，確認(rèn)無遺留火種后方可離開現(xiàn)場。5. 監(jiān)火人需在動火作業(yè)完成1小時及3小時內(nèi)，返回現(xiàn)場檢查確認(rèn)無遺留火種。Responsibility of fire watcher:1 Fire watcher must constantlyroam the work point that he is assigned for,aware for pote ntial hazards an d/or ch

The fire watcher shall be presented for at 1 hour and 3 hours after hot work isdon e.4.1.2動火作業(yè)安全要求 Safety requireme nt for hot work:4.1211.所有動火申請由本公司項目負(fù)責(zé)人負(fù)責(zé)，以“一天一證一動火點”為原則，負(fù)責(zé)現(xiàn)場安 全檢查和應(yīng)急 處理，熟悉消防器材使用方法；2動火人、監(jiān)火人都必須

15、熟悉動火要求，并承擔(dān)現(xiàn)場監(jiān)督、防火管理責(zé)任。3. 五級以上風(fēng)力或雨雪天氣，禁止室外動火作業(yè)；4. 所有金屬焊接、切割作業(yè)人員必須有u金屬焊接切割作業(yè)操作證”,為防止觸電事故的發(fā)生，動火人員必須佩戴焊工專用絕緣手套、絕緣鞋及其它防護(hù)用品，包括外服人員和外 來施工人員， 并保證焊機(jī)絕緣層無破損；5. 動火作業(yè)前必須將周圍易燃易爆物品移至安全位置。一般情況下，在施工區(qū)10m范圍 內(nèi)，無可燃物，易燃液體，易燃?xì)怏w，可燃粉塵等均清理干凈。如仍不能滿足安全要求，若半徑10m范圍內(nèi)的易燃、易爆品不能撤離，用防火布或濕木板隔離；（特別說明：靠近叉車電瓶區(qū)域作業(yè)時，必須將電瓶拆除或者切斷電瓶電源，并進(jìn)行充分通風(fēng)

16、，保證作業(yè)區(qū)域內(nèi)無燃爆 氣體后，方可動火，防止氫氣爆炸）6. 動火期間距離動火區(qū)域10米內(nèi)不得排放各類可燃?xì)怏w，可燃液體；在動火點10米范圍 內(nèi)及用火點下方不得同時進(jìn)行可燃溶劑清洗、噴漆或拋丸等作業(yè)。7. 所有氣瓶嚴(yán)禁靠近火源、熱源和電氣設(shè)備，與明火距離不少于5米；氧氣瓶和乙快氣瓶距離不少于5米。乙快瓶禁止使用其它氣瓶進(jìn)行替代，并保證有止回火閥。所有氣瓶必須無 破損，壓力正常；8.動火現(xiàn)場配置兩只以上滅火器，必要時將消防水帶鋪設(shè)到作業(yè)現(xiàn)場。無火警時禁止使用，否則照價賠 償，使用完成，及時放回指定位置；9.動火結(jié)束后，動火人員和監(jiān)火人須共同進(jìn)行確認(rèn)，保證現(xiàn)場無火警后，方可離開作業(yè)現(xiàn) 場。動火作業(yè)

Fire operator must be trained specially, and gets the license. They sh

Any damaged or frayed cord in weld ing machi ne should remove from service The con structors are required to follow this requireme nt.5. Exposed flammable/combustible materials and equipme nt with a 10m radium must remove

20、 or be protected by fire-proof covers or fire bla nkets before hot work begins.(Remark: Shut off the power and keep good ventilation while fire operati on being done n ear charg ing area, it will preve nt gas explosi on)6. All combustible dust,vapor, gas of liquids must be kept 10m away or properly

21、covered with fire-proof curtains, fire blan kets .It mea ns weld/cut within 10m was forbidde n while a paint booth or shot blaster was running.7. Oxyge n cyli nder should keep at least 5m away from acetyle ne cyli nder. Mean while gas cyli nder should keep 5m away from ign iti on source and in good

22、con diti on.8. Two or more fire ext in guishers must be kept in job site.9. Clean the job site when finished fire operation and make sure there is no fire risk. The fire watcher shall be presented for at 1 hour and 3 hours after hot work is done EHS departme nt will archive hot work permit.4.2高空作業(yè) W

23、ork at height4.1.1職責(zé) Resp on sibility4.1.1.1申請人的職責(zé)(必須由我司項目負(fù)責(zé)人擔(dān)當(dāng)):1. 高處作業(yè)前負(fù)責(zé)高處作業(yè)申請，并遵守“一天一證一作業(yè)點”的原則；2. 對外來施工單位的資格合法性審查，提醒外來施工單位帶齊高處作業(yè)所需要的相關(guān)證件、防護(hù)裝備、 簽訂施工商廠內(nèi)作業(yè)安全協(xié)議；以備我司檢查；3. 對高處作業(yè)人員及監(jiān)護(hù)人員進(jìn)行培訓(xùn)，確保熟悉高處作業(yè)要求；4. 每天上、下午不少于一次對作業(yè)現(xiàn)場進(jìn)行巡視；5. 要求作業(yè)方作業(yè)結(jié)束后，將現(xiàn)場整理整頓。Resp on sibility of applies nt (The applies nt should b

26、 orderly and clean.421.2會簽人的職責(zé)(使用部門主管擔(dān)當(dāng)):1. 對作業(yè)現(xiàn)場進(jìn)行確認(rèn)，確保現(xiàn)場符合高處作業(yè)要求；2協(xié)調(diào)并避免施工與生產(chǎn)之間的交叉作業(yè)；3.確認(rèn)高處作業(yè)下方3米范圍內(nèi)無其他作業(yè)。Supervisor of job site is resp on sible to:1 Check out the precautions before approving the operation; ensure that it ssafe to perform work.2. Coordin ate to preve nt daily work con flict with

Check out there is no work with a 3 meters radiom.421.3審核人的職責(zé)(安全部門擔(dān)當(dāng))；1 .高處作業(yè)前對作業(yè)現(xiàn)場進(jìn)行確認(rèn)，確?，F(xiàn)場符合作業(yè)要求；2檢查作業(yè)人員咼處作業(yè)申請單手續(xù)申請是否齊全、作業(yè)人員防護(hù)是否配齊；施工方針對所有高處 作業(yè)的管理要求是否了解；3.審核人員每天對施工現(xiàn)場上下午各不少于1次巡杳，發(fā)現(xiàn)異常立即予以糾正，并記錄。Resp on sibility of auditor (facility-EHS departme nt):1 Check out the condition of job

29、 on, shut dow n the operati on and take corrective acti ons.4.2.1.4作業(yè)人員的職責(zé)1.針對身體狀況負(fù)責(zé)，有咼處作業(yè)禁忌的癥狀（咼血壓、心臟病、貧血、癲癇等）視力不 佳、疲勞過度 等不適宜進(jìn)行高處作業(yè)的情況，嚴(yán)禁高處作業(yè)；2查驗咼處作業(yè)申請單的審批情況，無咼處作業(yè)申請單、申請單上相關(guān)流程未審 批完畢的應(yīng)禁 止作業(yè)；3. 掌握高處作業(yè)的安全要求，并逐項確認(rèn)相關(guān)安全措施的落實情況；4. 配齊防護(hù)設(shè)備，對于危險性過高且無法進(jìn)行安全保障的應(yīng)禁止作業(yè)；5. 高于5米的高處作業(yè)（3個腳手架高度）必須持證上崗；6. 確認(rèn)咼處作業(yè)地點和時間，是否

If operator is worki ng above 5m, he shall receive special tech no logy trai ning, pass releva nt test and gain authorizati on.6.
