Purpose目的To follow the procedure when a fire protection system is out of service so as to ensure the risks are minimized and the duration of the impairment is limited as well as the damage of the personnel and facility.通過嚴格控制消防系統(tǒng)中斷過程中各環(huán)節(jié)來減少以至于最終杜絕消防系統(tǒng)的失

Scope范圍This policy applies to all factory employees, contractors and their representatives to provide service when the fire system is impaired partially or totally.本程序適用于在工廠里工作的雇員，承包商或者他們的代表在工作過程中將要對當前使用的消防系統(tǒng)造成部分或全部功能中斷時。3. Definition定義Fire Protection System:

6、ent: When a fire system is out of service in whole or part due to a plan in advance such as revisions to the water supply or sprinkler piping system.l 計劃中斷：消防系統(tǒng)由于提前計劃規(guī)定在某段時間內(nèi)功能部分或者全部喪失，比如消防管網(wǎng)或者雨林系統(tǒng)的升級改造等。l Level 1 impairment: when a limited number of a component of a fire protection system are found

The director of the Healthy and Safety or the manager of the Department of the Environment will be the Impairment Coordinato

Responsibility 責任SE employees：All the employees who impair a fire protection system, in whole or part, must first notify the Impairment Coordinator at the Department of the Environmental Health and Safety. If the impairment permit

10、 is required, the employees will attach the Fire & Life Safety System Permit or Impairment Tag (schedule A) to fire department connection for the sprinklers system impairment or the control panel for the fire alarm systems, and then issue the Permit to Work License of Fire Control Facility based

11、 on the “Management Procedure of Safety Work Permit”.SE 雇員：任何一個雇員在工作過程中有可能全部或者部分中斷一個系統(tǒng)的消防功能時，必須第一時間報告環(huán)保健康安全部門的中斷協(xié)調(diào)員，經(jīng)研究如果確實需要消防系統(tǒng)中斷，則從中斷協(xié)調(diào)員那里申請一個防火逃生許可/中斷標牌（見計劃A），并將此防火逃生許可/中斷標牌掛在消防中斷區(qū)域的噴淋系統(tǒng)閥組或者是火災(zāi)報警盤上, 然后再按照安全工作許可證管理程序開具在消防設(shè)施上開展施工的作業(yè)許可證。Contractor or Outside Company Representatives:Any contractor o

12、r the Outside Company Representatives who impair a fire protection system, in whole or part, must first notify the Impairment Coordinator at the Department of the Environmental Health and Safety. If the impairment permit is required, any contractor or the Outside Company Representatives will attach

13、the Fire & Life Safety System Permit or Impairment Tag (schedule A), which would be a precondition to apply for work permit before starting the operation, to fire department connection for the sprinklers system impairment or the control panel for the fire alarm systems.合同商或者外部公司代表：任何合同商或者外公司代表在工

14、作過程中有可能全部或者部分中斷一個系統(tǒng)的消防功能時，必須第一時間報告環(huán)保健康安全部門的中斷協(xié)調(diào)員，如果確實需要消防系統(tǒng)中斷，需要從中斷協(xié)調(diào)員那里申請一個防火逃生許可/中斷標牌（見計劃A），并且將是否開具此標牌作為能否申請作業(yè)許可的前置條件，在開始作業(yè)施工前,并將此防火逃生許可/中斷標牌掛在需要中斷區(qū)域的噴淋系統(tǒng)閥組或者是火災(zāi)報警盤上。Impairment Coordinator(IC):1. IC will determine the level of the impairment due to a shutdown, in whole or part, of a fire protectio

15、n system;2. The IC will be responsible for the completion of the Impairment Worksheet (Schedule B) when authorizing Level 2 impairment.3. The IC will be responsible for issuing and verifying removal of fire &life safety system permit /impairment tags when authorizing Level 2 impairment.4. The IC

16、 will be responsible for coordination of the fire watch, if it has been determined that a fire watch is necessary. The IC will document the fire watch activities preparation or completion on Schedule C, the Fire watch Log Sheet.5. The IC will be responsible for providing training to SE employees中斷協(xié)調(diào)

Impairment Coordination Procedure-Level 2Preplanned Impairments:l The SE Dept. of the Public Safety will be notified of a preplanned im

20、pairment of a fire alarm system. l SE Security Dep. and the FFCC will be notified of the preplanned impairment of a fire alarm system.l The maintenance Supervisor for affected building will be notified of the preplanned impairment of a fire alarm system.l The impairment Coordinator will complete the

21、 Impairment Worksheet (Schedule B).l The impairment Coordinator will complete the fire&life safety system permit /impairment tag (Schedule A). l The impairment Coordinator will approve the applications tag of fire&life safety system permit /impairment whether the applicants complete or not.l

22、 The IC will authorize the preplanned impairment and verify that the following procedures have been implemented.1. The extent and expected scope of the impairment have been determined; 2. The areas or building involved have been considered and increased risks determined; 3. When a required automatic

23、 sprinkler system is out of service for an extended duration, the impairment coordinator will arrange for one of the following: a. Evacuation of the people in the building or portion of the building affected by the system out of service b. A fire watch for all portions of the building left unprotect

24、ed by the shutdown until the fire alarm or the sprinkler system has been returned back to service. Fire watch personnel used for the automatic sprinkler system shutdown must be specially trained in the fire prevention and the fire watch is responsible to take notes of the impairment effect. 4. When

25、a required fire alarm system is out of service for an extended duration, the impairment coordinator will arrange for one as the following: a. Evacuation of the people in the building or portion of the building affected by the system out of service b. Implementation of mitigating measures for the per

26、iod that the system is impaired. This considers the building occupancy type, nature and duration of impairment, building occupancy level during the impairment period, active work being conducted on the fire alarm system during the impairment, condition of the other fire protection systems and featur

27、es, and hazards and assets at risk. 5. Sprinkler system: the impairment coordinator will complete or authorize a Fire& Life Safety System Permit/ Impairment Tag (Schedule A), making sure it is located at the fire department connection, indicating which system, or part thereof, have been removed

28、from service. 6. Fire alarm system: the impairment coordinator will complete or authorize a Fire& Life Safety System Permit/ Impairment Tag (Schedule A), making sure it is located at fire alarm annunciator or the fire alarm control panel.6. 中斷協(xié)調(diào)程序2級l 每次有計劃的消防中斷都應(yīng)當通知SE負責公共安全的部門l 每次有計劃的消防中斷都應(yīng)當通知SE

29、安保部門或者FFCCl 每次有計劃的消防中斷都應(yīng)當通知受影響建筑物的維修負責人l 中斷協(xié)調(diào)員應(yīng)當填寫消防中斷表格（見計劃B）l 中斷協(xié)調(diào)員應(yīng)當簽發(fā)防火逃生許可/中斷標牌（見計劃A）l 中斷協(xié)調(diào)員在批準同意之前審核作業(yè)許可證申請者是否按照此程序規(guī)定完成防火/逃生許可中斷標簽的申請。l 中斷協(xié)調(diào)員授權(quán)實施有計劃的消防中斷并確保下述各項貫徹實施1) 消防中斷所預(yù)期持續(xù)的時間長度及其可能的范圍都已經(jīng)確定2) 消防中斷影響的區(qū)域及其建筑物以及可能增加的風險已經(jīng)確定3) 在受中斷影響區(qū)域的自動噴淋系統(tǒng)失效期間，消防中斷需要延長時間時，中斷協(xié)調(diào)員提前應(yīng)至少有下述的一項安排：a) 疏散受影響的區(qū)域或建筑物內(nèi)的

30、人員b) 應(yīng)安排看火員值班直到受影響區(qū)域消防系統(tǒng)恢復(fù)功能為至，接受過專門的培訓(xùn)的看火員在此期間應(yīng)巡視受影響區(qū)域并做好記錄4) 在受消防中斷影響區(qū)域的火災(zāi)報警系統(tǒng)失效期間，消防中斷需要延長時間時，中斷協(xié)調(diào)員提前應(yīng)至少有下述的一項安排：a) 疏散受影響的區(qū)域或建筑物內(nèi)的人員b) 在系統(tǒng)消防中斷期間實施適當?shù)臏p輕災(zāi)害措施，應(yīng)當考慮建筑物的類型，以及消防中斷可持續(xù)的時間，消防系統(tǒng)中斷期間火災(zāi)報警系統(tǒng)仍然在使用的功能，其他臨近消防系統(tǒng)的狀態(tài)及其特性以及資產(chǎn)所面臨的其他風險狀態(tài)等5) 噴淋系統(tǒng)：中斷協(xié)調(diào)員應(yīng)完成或者授權(quán)完成防火逃生許可/中斷標牌（見計劃A）的簽發(fā)，并要將此牌掛在受影響區(qū)域的消防連接處以此來

31、顯示是哪個部門，哪個系統(tǒng)的哪個部分被拆除或者是暫時處于失效狀態(tài)。6) 火災(zāi)報警系統(tǒng)：中斷協(xié)調(diào)員應(yīng)完成或者授權(quán)完成防火逃生許可/中斷標牌（見計劃A）并確保此標牌掛在受影響區(qū)域的火災(zāi)控制盤上。7. Emergency Impairment Procedure When an emergency impairment occurs, emergency action will be taken to minimize the potential injury and damage. l The IC will implement items 1 to 6 of section of Preplann

32、ed Impairment with the following exception:Any damage to the fire protection system caused by the contractor will be repaired by a licensed fire protection company at the contractors expense. Any required fire watch activities due to the damage of the fire protection equipment will be supplied by th

33、e contractor. Contractor must supply their own fire watch activities to ensure compliance with this policy and other SE policy. 7. 緊急中斷程序當緊急中斷發(fā)生時，采取應(yīng)急措施來減少潛在的人員傷害和財產(chǎn)損失。l 中斷協(xié)調(diào)員除了要執(zhí)行中斷協(xié)調(diào)程序2級中從1）到6）的所有步驟之外，還需要包括下列一條：n 凡由于承包商導(dǎo)致的消防系統(tǒng)的損壞都應(yīng)該讓有資質(zhì)的消防公司修復(fù)并且費用應(yīng)當由承包商負擔；并且在此期間的專職防火員應(yīng)當由承包商自己提供，承包商必須提供他們自己看火員的主要活動

Restoring System to Service:When all impaired equipment is restored to normal working order, the impairment coordinator will verify that the following procedures have been implemented:l Any necessary inspections and tests have been conducted to verify that affected systems are oper

35、ational. l Maintenance Supervisor for the affected building has been advised that the protection is resorted.l If a fire alarm system is involved, the SE Dept. of Public Safety and monitoring company (if any) have been advised that the protection is resorted.l The Fire&Life Safety System Permit

36、/Impairment Tag have been shut down and all the facilities have been advised that the protection is resorted.8. 恢復(fù)系統(tǒng)功能當一個系統(tǒng)恢復(fù)到正常的工作狀態(tài)時，中斷協(xié)調(diào)員要確保下列工作已經(jīng)完成。l 所有用來保證系統(tǒng)功能正常的測試和檢查都已經(jīng)完成。l 受影響區(qū)域或者建筑物的維修負責人已經(jīng)證實消防系統(tǒng)已經(jīng)恢復(fù)正常使用。l 在消防系統(tǒng)中斷期間，火災(zāi)報警也涉及的，SE負責公共安全的部門以及安保部門也已經(jīng)證實消防系統(tǒng)功能恢復(fù)正常使用。l 關(guān)于此次消防中斷的所有工作許可都已經(jīng)關(guān)閉，現(xiàn)場的設(shè)備恢復(fù)正
